摘要
急性髓细胞白血病(AML)是髓系造血干/祖细胞恶性疾病,多数患者病情危重,预后差。AML的治疗手段发展迅速,包括传统化疗、靶向治疗、免疫治疗、造血干细胞移植,且不同患者的细胞遗传学、分子生物学和临床表现异质性强,故需要个体化治疗。在精准治疗时代背景下,临床医生选择治疗方案的方式也在发生变化,本文就患者治疗综合成本、家庭经济支持系统和治疗期望为原则,评述基于医患共同决策(SDM)来探讨患方参与的临床决策模式和流程,以期待实现真正个体化治疗的目标。这不仅是治疗患者疾病,更是一种尊重患者治疗愿望的人文文化,值得临床高度重视与借鉴。
Acute myeloid leukemia(AML)is a malignant disease of myeloid hematopoietic stem/progenitor cells.Most patients with AML are in critical condition and most have a poor prognosis.Treatments for AML develop rapidly,including traditional chemotherapy,targeted therapy,immunotherapy,and hematopoietic stem cell transplantation.Moreover,the cytogenetic,molecular biology,and clinical manifestations of different patients are highly heterogeneous,so individual treatment is needed.In the era of precision therapy,the way clinicians choose treatment plans is also changing.In the hope of achieving the goal of truly individualized therapy,It has become a trend to explore the clinical decision making model and process that patients participate based on shared decision making(SDM)and the principle of comprehensive cost of treatment,family financial support system and treatment expectation of patients.

吴俣,教授,主任医师,博士生导师,四川大学华西医院血液科副主任/党支部书记,2014-2015年赴全美排名第二肿瘤中心美国纽约Memorial Sloan Kettering Cancer Center(MSKCC)学习,长期从事血液临床、教学、科研工作,担任日本免疫学会会员,美国血液学会国际会员,2010年起担任国家自然科学基金一审专家,2014年分别评为“第十一批四川省学术技术带头人后备人才”,“第十一批四川省卫生计生委学术技术带头人后备人才”,2016年获得“第十二批四川省卫生计生委学术技术带头人”,2020年获得“第十三批四川省学术技术带头人”,曾担任中华医学会血液学分会第九届青年委员会副主任委员,现任研究型医院学会血液病精准诊治专业委员会委员,中国抗癌协会MDS/MPN工作组副组长,中华医学会血液学分会白血病淋巴瘤组委员,中国抗癌协会髓系肿瘤学组委员,中华老年医学会MDS/MPN学组委员,四川省女医师协会血液学分会副主任委员,四川省医师协会第一届血液医师分会委员,四川省肿瘤协会血液肿瘤专委会副主任委员,先后在《Journal of Immunology》《Oncogene》《Journal of Clinical Investigation》《leukemia&lymphoma》发表SCI论文30多篇,获得国家自然科学基金、四川省科技厅科技支撑计划10多项,作为PI参加国际多中心临床研究多项,E-mail:wu_yu@scu.edu.cn,共同第一作者:张昕,E-mail:xinZx1995@163.com。
