摘要
目的观察星状神经节远程监护治疗系统预防非瓣膜房颤卒中的有效性和安全性。方法将2017年12月至2018年12月本院收治的66例非瓣膜性房颤患者按照信封随机法分为治疗组(n=33)和对照组(n=33),试验结束后剔除7例,最终治疗组有28例,对照组有31例。治疗组采用星状神经节远程监护治疗系统治疗,对照组采用华法林治疗。比较两组的终点结局指标和凝血指标。结果两组均未出现卒中、死亡、少量出血及以上程度的出血事件。两组的轻微出血、栓塞/血栓形成发生率比较,差异无统计学意义(P>0.05)。治疗后,治疗组的PT短于对照组,APTT长于对照组,INR低于对照组(P<0.01);两组的TT和FIB水平比较,差异无统计学意义(P>0.05)。结论在非瓣膜性房颤患者卒中防治中,星状神经节远程监护治疗系统与华法林的临床疗效无明显差异,但星状神经节远程监护治疗系统的出血风险低于华法林,验证了该系统预防非瓣膜房颤所引起脑卒中的安全性和有效性,具有临床推广价值。
Objective To observe the effectiveness and safety of stellate ganglion telemonitoring and treatment system in preventing stroke of non-valvular atrial fibrillation.Methods A total of 66 patients with non-valvular atrial fibrillation admitted in our hospital from December 2017 to December 2018 were divided into treatment group(n=33)and control group(n=33)according to the envelope random method.After the end of the trial,7 cases were excluded.There were 28 cases in the treatment group and 31 cases in the control group,finally.The treatment group was treated with telemonitoring and treatment system,and the control group was treated with warfarin.The endpoint outcome and coagulation indexes were compared between the two groups.Results There were no stroke,death,a small amount of bleeding and more bleeding events in both groups.There were no significant differences in the incidence of slight bleeding and embolism/thrombosis between the two groups(P>0.05).After treatment,PT in the treatment group was shorter than that in the control group,APTT was longer than that in the control group,and INR was lower than that in the control group(P<0.01);there were no significant differences in TT and FIB level between the two groups(P>0.05).Conclusion In the prevention and treatment of stroke in patients with non-valvular atrial fibrillation,there is no significant difference in the clinical efficacy between the stellate ganglion telemonitoring and treatment system and warfarin,but the bleeding risk of stellate ganglion telemonitoring and treatment system is lower than warfarin,which verifies the safety and effectiveness of the system in preventing stroke caused by non-valvular atrial fibrillation,and has clinical promotion value.
