摘要
目的 观察注射用辅酶Ⅰ治疗冠心病的临床效果与安全性及药物经济性。方法 选取2018年1月-2020年1月郑州大学第二附属医院收治的冠心病患者240例,依照随机数字表法分为观察组和对照组,各120例。在此基础上,对照组予注射用烟酸治疗,观察组采用注射用辅酶Ⅰ治疗。比较2组临床疗效,治疗前后心功能指标[脑钠肽(BNP)、超敏C反应蛋白(hs-CRP)、左心室射血分数(LVEF)]变化,药物成本—效果分析及不良反应(胃肠道反应、皮疹、肝肾功能异常等)发生情况。结果 观察组患者治疗总有效率为87. 50%,高于对照组的72. 50%(χ^(2)=8. 438,P=0. 004);治疗后2组患者BNP、hs-CRP水平均较治疗前降低,LVEF水平较治疗前升高(P均<0. 01),且观察组上述各指标降低或升高幅度大于对照组(P均<0. 01);观察组费用成本低于对照组;观察组不良反应总发生率为7. 50%,低于对照组的32. 50%(χ^(2)=23. 438,P=0. 000)。结论 注射用辅酶Ⅰ治疗冠心病的临床效果佳,安全性好,且相比于烟酸更有经济性优势,易为患者接受,值得临床推广应用。
Objective To observe the clinical effect,safety and drug economy of coenzyme Ⅰ for injection in the treatment of coronary heart disease.Methods Two hundred and forty patients with coronary heart disease admitted to the Second Affiliated Hospital of Zhengzhou University from January 2018 to January 2020 were selected.They were randomly divided into observation group and control group,120 cases in each group.The control group was treated with nicotinic acid for injection,and the observation group was treated with coenzyme Ⅰ for injection.The clinical efficacy,changes of cardiac function indexes(BNP,hs-CRP,LVEF)before and after treatment,drug cost-effectiveness analysis and adverse reactions(gastrointestinal reactions,rash,liver and kidney dysfunction,etc.)were compared between the two groups.Results The total effective rate of the observation group was 87.50%,which was higher than 72.50%of the control group(χ^(2)=8.438,P=0.004);After treatment,the levels of BNP and hs CRP in the two groups were lower than before treatment,and the level of LVEF was higher than before treatment(P<0.01),and the decrease or increase of the above indexes in the observation group was greater than that in the control group(P<0.01);The cost of observation group was lower than that of the control group;The total incidence of adverse reactions in the observation group was 7.50%,which was lower than 32.50%in the control group(χ^(2)=23.438,P=0.000).Conclusion The clinical effect and safety of coenzyme I for injection in the treatment of coronary heart disease is good,and compared with nicotinic acid,it has more economic advantages and is easy to be accepted by patients,so it is worthy of clinical application.
