摘要
目的评价健康教育对糖尿病患者血糖波动存在的影响。方法从该中心2020年1月—2021年5月管理糖尿病患者中选取104例作为研究对象,根据不同治疗方案分成对照组(52例,常规治疗)与观察组(52例,常规治疗+综合健康教育干预),对两组血糖变化、血糖波动变化、低血糖发生情况、治疗依从情况以及其他临床疗效评价指标进行观察比较。结果经护理观察组空腹血糖(6.57±1.19)mmol/L、餐后2 h血糖(7.86±1.36)mmol/L、糖化血红蛋白(6.89±1.77)%低于对照组(8.02±1.21)mmol/L、(10.54±2.44)mmol/L、(9.21±1.41)%,差异有统计学意义(P<0.05);血糖水平标准差(1.77±0.56)mmol/L、变异系数(0.18±0.05)、平均血糖波动幅度(3.77±0.57)mmol/L、四分位血糖波动范围(2.51±1.08)mmol/L、餐后血糖波动幅度(2.29±1.11)mmol/L低于对照组(2.59±1.33)mmol/L、(0.23±0.04)、(4.54±1.21)mmol/L、(3.95±1.05)mmol/L、(3.38±1.56)mmol/L,差异有统计学意义(P<0.05);低血糖发生率(3.85%)低于对照组(19.23%),治疗依从率(92.31%)高于对照组(76.92%),差异有统计学意义(χ^(2)=6.029、4.727,P<0.05);心理状态、睡眠质量评分(71.25±12.21)分、(8.17±2.69)分低于对照组(85.32±10.14)分、(10.36±2.24)分,自我效能感评分(33.27±6.19)分、生活质量评分(86.69±12.32)分高于对照组(28.58±6.36)分、(80.02±12.25)分,差异有统计学意义(P<0.05)。结论综合健康教育临床价值显著,不仅能够改善糖尿病患者血糖水平,降低血糖波动,也能够减少低血糖发生率,提高治疗依从性,调节心理状态,促进睡眠质量、自我效能感、生活质量提升。
Objective To evaluate the impact of health education on blood glucose fluctuations in diabetic patients.Methods From January 2020 to May 2021,104 patients with diabetes management in the center were selected as the study subjects,and divided into control group(52 cases,conventional treatment)and observation group(52 cases,conventional treatment+comprehensive health education intervention)according to different treatment regimen.3 The changes of blood glucose,blood glucose fluctuation,the occurrence of hypoglycemia,treatment compliance and other clinical efficacy evaluation indicators were observed and compared between the two groups.Results The fasting blood glucose(6.57±1.19)mmol/L,2 h postprandial blood glucose(7.86±1.36)mmol/L and glycosylated hemoglobin(6.89±1.77)%of the nursing observation group were lower than those of the control group(8.02±1.21)mmol/L,(10.54±2.44)mmol/L,(9.21±1.41)%,the difference was statistically significant(P<0.05);standard deviation of blood glucose level(1.77±0.56)mmol/L,coefficient of variation(0.18±0.05),average blood glucose fluctuation range(3.77±0.57)mmol/L,interquartile blood glucose fluctuation range(2.51±1.08)mmol/L,postprandial blood glucose fluctuation range(2.29±1.11)mmol/L was lower than the control group(2.59±1.33)mmol/L,(0.23±0.04),(4.54±1.21)mmol/L,(3.95±1.05)mmol/L,(3.38±1.56)mmol/L,the difference was statistically significant(P<0.05);the incidence of hypoglycemia(3.85%)was lower than that of the control group(19.23%),and the treatment compliance rate(92.31%)was higher than that of the control group(76.92%),the difference was statistically significant(χ^(2)=6.029,4.727,P<0.05);mental state,sleep quality score(71.25±12.21)points,(8.17±2.69)points lower than the control group(85.32±10.14)points,(10.36±2.24)points,self-efficacy sense score(33.27±6.19)points,quality of life score(86.69±12.32)points higher than the control group(28.58±6.36)points,(80.02±12.25)points,the difference was statistically significant(P<0.05).Conclusion Comprehensive health education has significant clinical value.It can not only improve the blood glucose level of diabetic patients,reduce blood glucose fluctuations,but also reduce the incidence of hypoglycemia,improve treatment compliance,adjust mental state,and promote sleep quality,self-efficacy,and quality of life.
