摘要
目的探讨经皮双通道内镜技术治疗腰椎间盘突出症的疗效及并发症情况。方法本研究纳入2020年11月~2021年3月在经皮双通道内镜下行腰椎间盘突出髓核摘除术的30例患者资料,记录患者手术时长、住院时间及并发症情况,术前、术后1个月、3个月及末次随访时的腰腿痛VAS评分、改良JOA评分,采用改良MacNab标准评价疗效。结果30例患者平均手术时间(85.0±28.1)min,平均住院(5.1±1.1)d,术后随访(5.0±1.4)个月;术中硬膜囊撕裂2例,术后椎间盘突出复发1例。患者术后1、3个月及末次随访时的VAS评分、改良JOA评分均较术前改善,差异有统计学意义(P<0.05)。末次随访时,按改良MacNab标准评价疗效:优24例,良4例,可1例,差1例,优良率93.3%。结论双通道内镜技术治疗腰椎间盘突出症的短期临床疗效满意。
Objective To investigate the efficacy and complications of unilateral biportal endoscopy(UBE)in the treatment of lumbar disc herniation(LDH).Methods The clinical data of 30 LDH patients undergoing percutaneous unilateral biportal endoscopic discectomy from November 2020 to March 2021 were included in this study.The operation time,hospital stay,VAS score,modified JOA score,modified MacNab criteria and complications were recorded to evaluate the clinical effect.Results The average operation time was(85.0±28.1)min,average hospital stay was(5.1±1.1)d,and postoperative follow-up was(5.0±1.4)months.There were 2 cases of intraoperative dural sac tear,and 1 case of postoperative disc herniation recurrence.The VAS score and modified JOA score were improved at postoperative 1 month,3 months and the last follow-up,the differences were statistically significant(P<0.05).At the last follow-up,according to the modified MacNab criteria,the clinical efficacy was excellent in 24 cases,good in 4 cases,fair in 1 case and poor in 1 case,with the excellent and good rate of 93.3%.Conclusion:The short-term outcome of UBE in the treatment of LDH is satisfactory.
