虚拟3D手术仿真在单髁置换术中的应用研究

摘要 目的探讨虚拟3D手术仿真在单髁置换术(UKA)中的应用价值。方法选择2019年1月至2020年12月赣州市人民医院收治的拟行UKA术治疗的30例膝关节内侧间室骨性关节炎(OA)患者,按照随机数字表法分为研究组(15例)和对照组(15例)。研究组采用虚拟3D手术仿真辅助UKA术,对照组采用常规UKA术。比较两组手术相关指标、手术前后影像指标、关节功能恢复情况。结果研究组患者术中失血量、术后引流量少于对照组,手术时间和术后住院时间短于对照组,术后伸膝关节间隙高度、膝关节内翻角度低于对照组,差异有统计学意义(P<0.05);术后,两组患者Tegner膝关节运动评分、Lysholm膝关节评分均高于本组术前,差异有统计学意义(P<0.05),术后,两组患者Tegner膝关节运动评分、Lysholm膝关节评分比较,差异无统计学意义(P>0.05)。结论虚拟3D手术仿真辅助UKA术可减小手术创伤,减少术中失血量和术后引流量,改善术后影像学指标,缩短术后恢复时间,值得临床推广。 Objective To explore the application value of virtual 3D surgical simulation in unicompartmental kenn arthroplasty(UKA).Methods Thirty patients with osteoarthritis(OA)of medial compartment of knee joint who were admitted to Ganzhou People′s Hospital from January 2019 to December 2020 for UKA were selected and divided into study group(15 cases)and control group(15 cases)according to random number table method.The study group was given virtual 3D surgical simulation assisted UKA,while the control group was given conventional UKA.Operation-related indexes,postoperative imaging indexes and joint function recovery were compared between the two groups.Results The intraoperative blood loss and postoperative drainage volume in the study group were less than those in the control group,the duration of operation and postoperative hospital stay in the study group were shorter than those in the control group,the height of knee joint extension space and the angle of knee varus in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The Tegner knee motion scores and Lysholm knee joint scores in both groups after surgery were higher than those before surgery,and the differences were statistically significant(P<0.05).The postoperative Tegner knee joint motion scores and Lysholm knee joint scores were not significantly different between the two groups(P>0.05).Conclusion Virtual 3D surgical simulation assisted UKA can reduce surgical trauma,reduce intraoperative blood loss and postoperative drainage volume,improve postoperative imaging indicators,and shorten postoperative recovery time,which is worthy of clinical promotion.
