摘要
目的评价某大型三甲综合医院各临床科室临床路径实施的相对效率,为医院管理者及政府部门提高临床路径管理效率提供参考依据。方法选取2020年1—6月20个临床科室作为决策单元,运用数据包络分析(DEA)和灰色关联度对其临床路径的实施效率进行评价及分析。结果20个临床科室临床路径综合效率平均值为0.746,规模效率平均值为0.827,纯技术效率平均值为0.875;7个临床科室达到DEA相对有效,仅占35%;其中,病种数对临床路径实施效率影响最大,其次是覆盖率,影响最小的是变异率。结论医院20个临床科室临床路径的实施效率总体水平偏低,非DEA有效的临床科室存在投入冗余或产出不足现象,建议医疗机构应适时调整临床路径病种,细化和完善在病种上的选择,并通过加强临床路径信息化建设、完善临床路径绩效管理等措施,优化临床路径资源配置,提高临床路径的实施效率。
Objective To evaluate the relative efficiency of clinical pathway implementation in each clinical department of a large comprehensive third-grade A hospital,in order to provide references for hospital managers and government departments to improve the efficiency of clinical pathway management.Methods Twenty clinical departments from January.2020 to June.2020 were selected to be decision-making units.The relative efficiency was evaluated and analyzed using the DEA model and Grey Relation Analysis.Results The average clinical pathway comprehensive efficiency was 0.746,the average scale efficiency was 0.827,and the average pure technical efficiency was 0.875.The DEA of 7 DMUs achieved DEA relatively effectively,accounting for only 35%;among them,the number of diseases had the greatest influence on the implementation efficiency of clinical pathway,followed by the coverage rate,and the least influence was the variation rate.Conclusion Twenty clinical departments in the implementation of clinical pathway efficiency overall level were low,the non-DEA effective clinical departments had the phenomenon of redundant input or insufficient output,suggested that medical institutions should timely adjust clinical pathway diseases,refine and improve the selection of diseases,optimize the allocation of the clinical pathway resources and improve the implementation efficiency of clinical pathway by strengthening the information construction of clinical pathway and improving clinical pathway performance management.
