摘要
目的:探讨针灸联合不同穴位按摩在腹腔镜胆囊切除术后老年患者中的应用效果及对胃肠功能指标的影响。方法:选取2019年10月-2021年1月本院收治的86例行腹腔镜胆囊切除术的老年患者,按照患者入院顺序将其分为对照组和观察组,每组43例。两组均行腹腔镜胆囊切除术和对症治疗,对照组术后采用常规方式恢复患者的机体功能,观察组采用针灸及不同穴位按摩恢复患者的机体功能。比较两组疗效、术后胃肠功能恢复情况及术前和术后1、3 d血清胃肠激素水平[胃泌素(GAS)、胃动素(MTL)、血管活性肠肽(VIP)]。结果:观察组总有效率为93.02%,高于对照组的74.42%,差异有统计学意义(P<0.05)。观察组进食、肠鸣音恢复、肛门排气、下床活动、排便时间均短于对照组,差异均有统计学意义(P<0.05)。术前,两组GAS、MTL及VIP水平比较,差异均无统计学意义(P>0.05);术后1、3 d,观察组GAS、MTL均高于对照组,VIP低于对照组,差异均有统计学意义(P<0.05)。结论:将针灸和穴位按摩应用于腹腔镜胆囊切除术后老年患者的恢复中,可提高疗效,促进胃肠功能的恢复,改善排便功能。
Objective:To investigate the effect of acupuncture combined with acupoint massage in elderly patients after laparoscopic cholecystectomy (LC) and its influence on gastrointestinal function indexes.Method:A total of 86 elderly patients undergoing laparoscopic cholecystectomy admitted to our hospital from October 2019 to January 2021 were selected,and they were divided into control group and observation group according to the order of admission,43 cases in each group.Both groups were treated with laparoscopic cholecystectomy and symptomatic treatment,the control group was treated with conventional methods to recover the body function,and the observation group was treated with acupuncture combined with acupoint massage to recover the body function.The therapeutic effect,postoperative gastrointestinal function recovery were compared between two groups,and serum gastrointestinal hormone levels[gaolin (GAS),motilin (MTL) and vasoactive intestinal peptide (VIP)]before surgery and 1 and 3 d after surgery were compared between two groups.Result:The total effective rate of the observation group was 93.02%,which was higher than 74.42% of the control group,the difference was statistically significant (P<0.05).Eating,bowel sound recovery,anal exhaust,out of bed activity and defecation time of the observation group were shorter than those of the control group,the differences were statistically significant (P<0.05).Before surgery,there were no significant differences in GAS,MTL and VIP levels between two groups (P>0.05);at 1 and 3 d after surgery,GAS and MTL of the observation group were higher than those of the control group,and VIP of the observation group was lower than that of the control group,the differences were statistically significant (P<0.05).Conclusion:Acupuncture combined with acupoint massage applied to the recovery of elderly patients after LC can improve the efficacy,promote the recovery of gastrointestinal function,improve the function of defecation.
