摘要 目的:探讨T_(1) mapping技术在评价肥厚型心肌病(HCM)患者心肌整体纤维化中的应用价值,并分析HCM患者心肌纤维化程度与心脏功能的关系。方法:收集32例HCM患者为HCM组,招募17例正常者为对照组;2组均行心脏电影及T_(1) mapping序列检查,测量并比较2组左室心肌初始T_(1)值、心脏功能的差异;通过构建ROC曲线,计算心肌初始T_(1)值对2组的鉴别诊断效能;并对左室心肌初始T_(1)值与左室心肌质量、舒张末期容积行相关性分析。结果:HCM组的左室心肌初始T_(1)值[(1249.32±85.44)ms]高于对照组[(1042.35±78.80)ms],差异有统计学意义(P<0.05)。心肌初始T_(1)值鉴别HCM组与对照组的敏感度及特异度分别为87.5%、94.1%。HCM组左室舒张末期容积、收缩末期容积分别为(64.79±7.83)、(17.84±1.92)mL/m^(2),均低于对照组(均P<0.05);HCM组左室射血分数、心肌质量分别为(65.04±10.41)%、(107.64±18.96)g/m^(2),均高于对照组,差异均有统计学意义(均P<0.05)。HCM组左室心肌初始T_(1)值与左室心肌质量呈正相关(r=0.82,P<0.001),与左室舒张末期容积呈负相关(r=-0.68,P<0.001)。结论:T_(1) mapping技术在评价HCM患者心肌整体纤维化方面诊断价值较大;心肌纤维化程度与心脏功能存在一定相关性。 Objective:To explore the application value of T_(1) mapping technique in patients with hypertrophic cardiomyopathy (HCM),and to analyze the relationship between the degree of cardiac fibrosis and cardiac function in HCM patients. Methods:32 patients with HCM (the HCM group) and 17 healthy cases (the control group) were enrolled. Cardiac cinema and T_(1) mapping were performed. The initial T_(1) value of left ventricular myocardium and cardiac function between the two groups were measured and compared. The ROC curve was constructed to calculate the diagnostic value of the initial T_(1) value. The relationship among the initial T_(1) value of left ventricular myocardium and left ventricular mass,the end-diastolic volume were analyzed. Results:The initial T_(1) value of myocardium in the HCM group was higher than that in the control group [(1 249.32±85.44)ms vs. (1 042.35±78.80)ms],the difference was statistically significant (P<0.05). The sensitivity and specificity of initial T_(1) value in diagnosis to HCM were 87.5% and 94.1%,respectively. The left ventricular end-diastolic volume and end-systolic volume of the HCM group were (64.79±7.83) and (17.84±1.92)mL/m^(2),which were lower than those of the control group (both P<0.05). The left ventricular ejection fraction and myocardial mass of the HCM group were (65.04±10.41)% and (107.64±18.96)g/m^(2),which were statistically significant higher than those of the control group (both P<0.05). The initial T_(1) value of left ventricular myocardium in the HCM group was positively correlated with left ventricular mass (r=0.82,P<0.001) and was negatively correlated with the end-diastolic volume (r=-0.68,P<0.001). Conclusions:T_(1) mapping has greatly significant diagnostic value in evaluating the overall fibrosis of HCM patients. There is a certain correlation between the degree of cardiac fibrosis and cardiac function.
