摘要
目的:分析2012-2018年武汉市卫生资源配置公平性与效率的时空特征,以期对武汉市区域卫生资源合理布局提供参考。方法:运用基尼系数与集聚度分析卫生资源配置公平性,运用DEA-Windows分析卫生资源配置效率,运用空间统计方法分析卫生资源配置公平性和效率的空间特征。结果:武汉市卫生资源按地理面积和人口配置的基尼系数分别在0.52-0.76和0.09-0.41之间;中心城区卫生资源集聚度基本大于1,且呈下降趋势,新城区集聚度基本小于1;卫生资源配置呈逐年下降趋势的空间正相关;卫生资源配置综合效率、纯技术效率和规模效率分别在0.77-0.86、0.84-0.90和0.90-0.96之间。结论:武汉市卫生资源总量增加,结构欠均衡;物力资源公平性改善明显;按地理面积配置的公平性低于按人口配置的公平性;卫生资源配置呈逐步减弱的空间集聚特点;卫生资源配置综合效率较低,总体上升且空间差异显著,纯技术效率较低是其主要制约因素。
Objective:To analyze the spatio-temporal characteristics of equity and efficiency of health resource allocation in Wuhan from 2012 to 2018,so as to provide references for rational distribution of regional health resources in Wuhan.Methods:The Gini coefficient and agglomeration degree were used to analyze the equity of health resource allocation.The DEA-Windows method was used to analyze the allocation efficiency of health resources.The spatial statistics was used to analyze the spatial characteristics of equity and efficiency of health resource allocation.Results:The Gini coefficients of health resource allocation in Wuhan were 0.52-0.76 and 0.09-0.41 according to geographical area and population respectively.The agglomeration degrees of health resources in the central urban area were basically greater than 1 and showed a downward trend,and the agglomeration degrees of health resources in the new urban area were basically less than 1.The allocation of health resources showed a spatial positive correlation with a yearly decreasing trend.The comprehensive efficiency,pure technical efficiency and scale efficiency of health resource allocation were 0.77-0.86,0.84-0.90 and 0.90-0.96,respectively.Conclusion:The total amount of health resources in Wuhan has increased,but the structure is unbalanced.The equity of material resources has been significantly improved.The equity of allocation by geographical area is lower than that of allocation by population.The distribution of health resources shows the characteristic of gradually decreasing spatial agglomeration.The overall efficiency of health resource allocation is low but increasing.The spatial difference is significant.The lack of pure technical efficiency is the main restricting factor.
