摘要
目的探讨不明原因习惯性流产(unexplained recurrent spontaneous abortion,URSA)患者外周血中Th17/Treg细胞平衡及炎症因子(IL-7和IL-10)水平,并分析其相关性,为URSA的临床预防和治疗提供参考依据。方法URSA组来源于2018年2月—2020年6月该院收治的URSA患者184例,对照组来源于正常早孕组和正常未孕组各95例。采用流式细胞仪测定URSA组和对照组外周血中Th17细胞、Treg细胞和Th17/Treg细胞比值,采用ELISA法测定血清中炎症因子(IL-7和IL-10)的表达水平,并采用Pearson相关分析URSA组中Th17/Treg比值与IL-7和IL-10的相关性。结果URSA组中Th17细胞、Th17/Treg比值明显高于早孕组和未孕组,Treg细胞明显低于早孕组和未孕组[Th17(%):(2.94±0.68)vs.(1.33±0.26)和(1.43±0.24);Th17/Treg:(0.85±0.14)vs.(0.29±0.05)和(0.31±0.04);Treg(%):(3.56±0.79)vs.(4.78±1.13)和(4.87±1.07);均P<0.01],差异有统计学意义。而早孕组和未孕组中3种指标差异无统计学意义(均P>0.05)。URSA组中IL-17表达水平明显高于早孕组和未孕组,IL-10表达水平明显低于早孕组和未孕组[IL-17(pg/ml):(39.32±6.34)vs.(18.75±4.02)和(21.53±4.23);IL-10(ng/ml):(73.84±18.21)vs.(185±21.42)和(176±19.53);均P<0.01],差异有统计学意义,而未孕组和早孕组中IL-17和IL-10表达差异无统计学意义(均P>0.05)。相关性分析显示,URSA组中Th17/Treg比值与IL-17呈现正相关性(r=0.796,P<0.01),而Th17/Treg比值与IL-10呈现负相关性(r=-0.653,P<0.01),差异有统计学意义。结论Th17/Treg、IL-10的高表达和IL-17的低表达与URSA发生有关。因此,Th17/Treg细胞平衡和炎症因子释放是导致URSA发生的原因之一。
Objective To explore the balance of Th17/Treg cell balance and inflammatory factors(IL-7 and IL-10)in the peripheral blood of patients with unexplained recurrent spontaneous abortion(URSA),and analyze their correlation to provide clinical prevention and treatment for URSA Reference.Methods The URSA group was derived from one hundred and eighty-four URSA patients admitted to Tangshan Maternal and Child Health Care Hospital from February 2018 to June 2020.The control group was derived from 95 cases in each of the normal early pregnancy group and the normal non-pregnancy group.Flow cytometry was applied to determine the ratio of Th17 cells,Treg cells and Th17/Treg cells in the peripheral blood of the URSA group and the control group.The expression levels of inflammatory factors(IL-7 and IL-10)in the serum were measured by ELISA.Pearson Correlation analysis was used to detect the correlation between Th17/Treg ratio and IL-7 and IL-10 in the URSA group.Results Th17 cells and Th17/Treg ratio in the URSA group were significantly higher than those in the early and non-pregnant group,and Treg cells were significantly lower than those in the early and non-pregnant group[Th17(%):(2.94±0.68)vs.(1.33±0.26)and(1.43±0.24);Th17/Treg:(0.85±0.14)vs.(0.29±0.05)and(0.31±0.04);Treg(%):(3.56±0.79)vs.(4.78±1.13)and(4.87±1.07);all P<0.01],the difference was statistically significant while the three indicators in the early pregnancy group and the non-pregnant group the difference was not statistically significant(all P>0.05).The expression level of IL-17 in URSA group was significantly higher than that in early pregnancy group and non-pregnancy group,and the expression level of IL-10 was significantly lower than that in early pregnancy group and non-pregnancy group[IL-17(pg/ml):(39.32±6.34)vs(18.75±4.02)and(21.53±4.23);IL-10(ng/ml):(73.84±18.21)vs(185±21.42)and(176±19.53);all P<0.01],the difference was statistically significant but there was no statistical difference in the expression of IL-17 and IL-10 between the non-pregnant group and the early pregnancy group Academic significance(all P>0.05).Correlation analysis showed that the Th17/Treg ratio and IL-17 in the URSA group showed a positive correlation(r=0.796,P<0.01),while the Th17/Treg ratio and IL-10 showed a negative correlation(r=-0.653,P<0.01)the difference was statistically significant.Conclusion The high expression of Th17/Treg and IL-10 and the low expression of IL-17 are related to the occurrence of URSA.Therefore,the balance of Th17/Treg cells and the release of inflammatory factors is one of the reasons leading to the occurrence of URSA.
