摘要
目的对比气管插管与喉罩麻醉对老年腹腔镜手术患者血流动力学指标与应激指标的影响。方法选择2018年5月—2020年4月该院收治的老年腹腔镜手术患者110例,以随机数表法分为对照组与研究组,各55例。对照组采用气管插管麻醉,研究组采用喉罩麻醉。对比两组不同时间点血流动力学指标与应激指标的变化。结果研究组喉罩置入或插管后即刻、喉罩置入或插管后5 min时平均动脉压(86.50±4.81)、(83.20±5.65)mmHg,心率(82.56±4.64)、(77.22±5.66)次/min均低于对照组(98.62±5.60)、(89.60±4.62)mmHg,(91.66±5.65)、(82.36±8.62)次/min,差异有统计学意义(t=12.176、6.503、9.231、3.697,P<0.001)。研究组喉罩置入或插管后即刻、喉罩置入或插管后5 min时去甲肾上腺素(1.19±0.20)、(1.20±0.15)nmol/L,皮质醇(400.65±46.85)、(380.52±58.65)ng/mL均低于对照组(1.32±0.20)、(1.31±0.22)nmol/L,(440.65±45.60)、(418.65±65.65)ng/mL,差异有统计学意义(t=3.409、3.064、4.537、3.213,P<0.05)。结论相较于气管插管,喉罩麻醉对老年腹腔镜手术患者血流动力学指标与应激指标的影响更轻微。
Objective To compare the effects of tracheal intubation and laryngeal mask anesthesia on hemodynamic and stress indexes in elderly patients undergoing laparoscopic surgery.Methods A total of 110 elderly patients undergoing laparoscopic surgery in the hospital from May 2018 to April 2020 were selected and divided into the control group and the study group with 55 cases each by random number table.The control group was anesthetized with tracheal intubation,and the study group was anesthetized with a laryngeal mask.Compared the changes of hemodynamic indexes and stress indexes between the two groups at different time points.Results In the study group,immediately after laryngeal mask insertion or intubation,5 min after laryngeal mask insertion or intubation,the average arterial pressure(86.50±4.81)mmHg,(83.20±5.65)mmHg,heart rate(82.56±4.64)beats/min,(77.22±5.66)times/min,lower than the control group(98.62±5.60)mmHg,(89.60±4.62)mmHg,(91.66±5.65)times/min,(82.36±8.62)times/min,the difference was statistically significant(t=12.176,6.503,9.231,3.697,P<0.001).Immediately after laryngeal mask insertion or intubation,at 5 min after laryngeal mask insertion or intubation,in the study group,norepinephrine(1.19±0.20)nmol/L,(1.20±0.15)nmol/L,cortisol(400.65±46.85)ng/mL,(380.52±58.65)ng/mL,lower than the control group(1.32±0.20)nmol/L,(1.31±0.22)nmol/L,(440.65±45.60)ng/mL,(418.65±65.65)ng/mL,the difference was statistically significant(t=3.409,3.064,4.537,3.213,P<0.05).Conclusion Compared with tracheal intubation,laryngeal mask anesthesia has a lesser effect on hemodynamic indexes and stress indexes in elderly patients undergoing laparoscopic surgery.
