MRI动态增强扫描联合DWI对良性前列腺增生与前列腺癌的鉴别分析 被引量:5

摘要 目的探讨磁共振成像(MRI)动态增强扫描联合弥散加权成像(DWI)对良性前列腺增生与前列腺癌的鉴别诊断价值。方法选取2018年1月至2020年12月于信阳市中心医院接受诊疗的86例前列腺肿瘤患者为研究对象,所有入选者均接受MRI动态增强扫描联合DWI扫描,以组织病理学检查为金标准,分析MRI动态增强扫描联合DWI对良性前列腺增生与前列腺癌的鉴别诊断价值。结果经组织病理学检查结果显示,86例前列腺肿瘤患者良性前列腺增生17例(19.77%),前列腺癌69例(80.23%);前列腺癌组Ⅱ型TIC曲线占比低于前列腺增生组,Ⅲ型TIC曲线占比高于前列腺增生组,差异有统计学意义(P<0.05);前列腺癌组T_(max)短于前列腺增生组,SER、MSI高于前列腺增生组,差异有统计学意义(P<0.05);前列腺癌组eADC值高于前列腺增生组,ADC值低于前列腺增生组,差异有统计学意义(P<0.05);绘制ROC曲线,结果显示,T_(max)、SER、MSI、eADC值、ADC值及联合鉴别良性前列腺增生与前列腺癌的AUC>0.7,具有一定诊断价值,其中联合诊断价值最高。结论MRI动态增强扫描联合DWI在良性前列腺增生与前列腺癌中具有较高鉴别诊断价值,可获得全面参数信息,提高诊断准确度。 Objective To investigate the value of dynamic contrast-enhanced magnetic resonance imaging(MRI)combined with diffusion-weighted imaging(DWI)in the differential diagnosis of benign prostatic hyperplasia and prostate cancer.Methods A total of 86 patients with prostate cancer who received diagnosis and treatment in Xinyang Central Hospital from January 2018 to December 2020 were selected as the research subjects.All selected patients underwent dynamic contrast-enhanced MRI combined with DWI.Histopathological examination was used as the gold standard to analyze the value of dynamic contrast-enhanced MRI combined with DWI in the differential diagnosis of benign prostatic hyperplasia and prostate cancer.Results By histopathological examination,the results showed that among 86 cases of patients with prostate cancer,17 cases(19.77%)were benign prostatic hyperplasia and 69 cases(80.23%)were prostate cancer.The proportion of typeⅡTIC curve in prostate cancer group was lower than that in benign prostatic hyperplasia group,and the proportion of typeⅢTIC curve was higher than that in benign prostatic hyperplasia group,with statistical significant differences(P<0.05).T_(max) of prostate cancer group was shorter than that of benign prostatic hyperplasia group,SER and MSI were higher than those of benign prostatic hyperplasia group,and the difference was statistically significant(P<0.05).eADC value of prostate cancer group was higher than that of benign prostatic hyperplasia group,and ADC value was lower than that of benign prostatic hyperplasia group,with statistical significant differences(P<0.05).The ROC curve was drawn,and the results showed that the AUC of T_(max),SER,MSI,eADC value,ADC value and their combination in differentiation between benign prostatic hyperplasia and prostate cancer were>0.7,which had certain diagnostic value,and the combined diagnostic value was the highest.Conclusion Dynamic contrast-enhanced MRI combined with DWI has high value in the differential diagnosis of benign prostatic hyperplasia and prostate cancer,comprehensive parameter information can be obtained to improve the diagnostic accuracy.
