摘要
目的探讨基于表面肌电图评估理筋手法治疗慢性非特异性下腰痛(CNLBP)患者肌肉功能恢复情况的价值。方法选取106例经理筋手法治疗的CNLBP患者作为研究对象,比较治疗前与治疗12周后患者的疼痛评分(VAS)、Roland-Morris功能障碍评分;统计治疗总有效率;比较治疗有效与无效患者椎旁肌群表面肌电指标;分析椎旁肌群表面肌电指标评价肌肉功能恢复情况的效能。结果治疗12周后,患者的VAS、Roland-Morris功能障碍评分均低于治疗前(P<0.05);治疗12周后的总有效率为83.96%;治疗后,治疗有效患者的多裂肌平均振幅、最大振幅以及竖脊肌平均振幅、最大振幅均高于无效患者(P<0.05);多裂肌平均振幅和最大振幅评估理筋手法治疗CNLBP患者肌肉功能恢复有效的曲线下面积(AUC)分别为0.812和0.806,竖脊肌平均振幅和最大振幅评估肌肉功能恢复有效的AUC分别为0.849和0.792。结论理筋手法治疗CNLBP,患者多裂肌和竖脊肌的表面肌电图指标均发生明显变化,表明其可作为评估患者肌肉功能恢复的客观指标。
Objective To explore the value of evaluating the muscle function recovery based on surface electromyography in patients with chronic non-specific low back pain(CNLBP)treated by manipulation for restoring and treating injured soft tissues.Methods A total of 106 patients with CNLBP treated by manipulation for restoring and treating injured soft tissues were selected as the research objects.The pain score(VAS)and Roland-Morris dysfunction score were compared before and 12 weeks after treatment;the total effective rate of treatment was counted;the surface electromyography indexes of paravertebral muscle group were compared between effective and ineffective patients;the surface electromyography indexes of paravertebral muscles were analyzed to evaluate the muscle function recovery.Results After 12 weeks of treatment,the VAS and Roland-Morris dysfunction scores of patients were lower than those before treatment(P<0.05);after 12 weeks of treatment,the total effective rate was 83.96%;after treatment,the average amplitude and maximum amplitude of the multifidus muscle and the erector spinal muscle in the effective patients were higher than those in the ineffective patients(P<0.05);for effective muscle function recovery in CNLBP patients,the area under the curve(AUC)of average amplitude and maximum amplitude of the multifidus muscle were 0.812 and 0.806,respectively,and the AUC of average amplitude and maximum amplitude of the erector spinal muscle were 0.849 and 0.792,respectively.Conclusion In the treatment of CNLBP with manipulation for restoring and treating injured soft tissues,the surface electromyography indexes of multifidus muscle and erector spinal muscle have obvious changes,indicating that they can be used as an objective indexes to evaluate the recovery of muscle function.
