摘要
目的观察分析急诊中乌拉地尔对于急性左心衰竭的临床治疗效果。方法80例急性左心衰竭患者,根据随机数字抽签法分为实验组和参照组,每组40例。参照组实施常规对症治疗联合硝酸甘油治疗,实验组实施常规对症治疗联合乌拉地尔治疗。观察比较两组患者给药前以及给药后30 min、1 h、2 h、12 h的心率、血压,治疗前和治疗48 h后的左室射血分数(LEVF)、左心室收缩末内径(LVESD)、B型钠尿肽(BNP)。结果实验组患者给药后30 min、1 h、2 h、12 h的心率分别为(100.23±5.31)、(88.91±5.13)、(82.33±5.48)、(80.57±5.46)次/min,均低于参照组的(111.54±7.65)、(105.11±7.29)、(103.45±5.67)、(103.11±7.46)次/min,差异有统计学意义(P<0.05);实验组给药30 min、1 h、2 h、12 h的血压分别为(125.82±10.14)、(118.45±8.67)、(114.15±7.89)、(111.16±7.69)mm Hg(1 mm Hg=0.133 kPa),均低于参照组的(143.12±14.07)、(140.14±12.89)、(136.56±13.14)、(135.44±13.82)mm Hg,差异有统计学意义(P<0.05)。实验组患者给药后48 h的LVEF(0.31±0.01)、LVESD(12.55±0.57)mm、BNP(197.78±40.14)ng/L均优于参照组的(0.19±0.03)、(19.91±0.41)mm、(300.31±67.51)ng/L,差异均具有统计学意义(P<0.05)。结论在急诊急性左心衰竭患者的临床给药中,实施乌拉地尔结合常规对症治疗相较于常规的硝酸甘油具备更为理想的效果,其能够更为显著的改善患者的心功能指标,值得临床推广。
Objective To observe and analyze the clinical effect of urapidil in the treatment of acute left ventricular failure.Methods A total of 80 patients with acute left ventricular failure were divided into an experimental group and reference group according to random numerical table,with 40 cases in each group.The reference group was treated with conventional symptomatic therapy and nitroglycerin,and the experimental group was treated with symptomatic therapy and urapidil.The heart rate and blood pressure before medication,and 30 min,1 h,2 h and 12 h after medication,left ventricular ejection fraction(LEVF),left ventricular end-systolic diameter(LVESD),brain natriuretic peptide(BNP)before treatment and 48 h after treatment were observed and compared between the two groups.Results 30 min,1 h,2 h and 12 h after medication,the heart rate of the experimental group were(100.23±5.31),(88.91±5.13),(82.33±5.48)and(80.57±5.46)beats/min,which were lower than(111.54±7.65),(105.11±7.29),(103.45±5.67)and(103.11±7.46)beats/min of the reference group,and the difference was statistically significant(P<0.05).30 min,1 h,2 h and 12 h after medication,the blood pressure were(125.82±10.14),(118.45±8.67),(114.15±7.89)and(111.16±7.69)mm Hg(1 mm Hg=0.133 kPa),which were lower than(143.12±14.07),(140.14±12.89),(136.56±13.14)and(135.44±13.82)mm Hg of the reference group,and the difference was statistically significant(P<0.05).48 h after medication,the LVEF,LVESD and BNP of the experimental group were(0.31±0.01),(12.55±0.57)mm and(197.78±40.14)ng/L,which were all better than(0.19±0.03),(19.91±0.41)mm and(300.31±67.51)ng/L of the reference group,and the difference was statistically significant(P<0.05).Conclusion Combination of urapidil and conventional symptomatic therapy has more ideal effect on emergency treatment of acute left ventricular failure compared with nitroglycerin,and it can significantly improve the cardiac function indicators of patients,which is worthy of clinical promotion.
