摘要
目的挖掘国医大师治疗瘿病验案中的用药规律,传承国医大师用药经验,为瘿病的临床用药提供参考。方法收集整理国医大师治疗瘿病验案中符合纳入标准的处方,提取医案中的中药处方数据,利用EXCEL 2013软件建立方药数据库,利用古今医案云平台(V2.2.3)集成的数据挖掘功能对中药数据进行频次分析、属性分析、关联分析、聚类分析等多角度统计分析。结果共纳入国医大师治疗瘿病验案中的处方共51首,涉及中药161味,总频次755次;用药频次>20次的中药依次为柴胡、牡蛎、甘草、浙贝母、郁金、夏枯草、白芍、连翘、玄参、茯苓;药物性味以苦寒为主;归经主要归属于肝经;药物功效主要为清热解毒、清热凉血、疏肝解郁、燥湿化痰等;药物关联分析排名前三为柴胡-郁金,浙贝母-连翘,柴胡-姜黄等;并通过聚类分析挖掘4组中药聚类方。结论国医大师多以肝论治瘿病,用药以苦寒药物为主,以清肝、化痰、祛瘀为治疗大法。
Objective To explore the medicine using law of traditional Chinese medicine master treating gall disease, inherit the experience of traditional Chinese medicine master, and provide reference for the clinical application of gall. Methods By reviewing the effective prescriptions for treating gall disease by in the cases of traditional Chinese medicine master’s, extract the traditional Chinese medicine prescription data in the medical cases, and use EXCEL 2013 software to establish the prescription database. Use of ancient and modern basis cloud platform(V2.2.3) integrated data mining function of traditional Chinese medicine data frequency analysis, attribute analysis, correlation analysis, clustering analysis and so on aspects of statistical analysis. Results A total of 51 prescriptions were included for treating gall disease by Chinese medicine masters, involving 161 traditional Chinese medicines and 755 times in total.The traditional Chinese medicine with a frequency of more than 20 times was bupleurum root, oyster, licorice root, fritillaria thunbergil, turmeric, prunella vulgaris, paeonia lactiflora, forsythia, radix ginseng, and poria cocos.The medicinal flavor was mainly bitter cold;the return to meridian is mainly attributed to the liver meridian;the main functions of the drugs are clearing away heat and detoxification, clearing heat and cooling blood, relieving depression in liver, dampness and phlegm, etc. The top three drug association analyses were Bupleurum-Turmeric, Fritillary thunberg-Forsythia, Bupleurum-turmeric, etc. And through cluster analysis, four groups of traditional Chinese medicine clustering formula were mined. Conclusion Chinese medicine masters treat gall disease based on liver, mainly with bitter cold medicine, and treat liver clearing, phlegm resolving and blood stasis removing as the general methods.
