摘要
目的分析防己黄芪汤合真武汤加减对气虚血瘀型慢性心力衰竭(CHF)患者的疗效。方法选取2018年2月至2019年1月开封市祥符区中医院收治的74例气虚血瘀型CHF患者作为研究对象,按照治疗方法分为常规组(37例)和观察组(37例)。常规组接受呋塞米片、马来酸依那普利片联合地高辛片治疗,观察组在对照组基础上加用防己黄芪汤合真武汤加减。评价两组疗效,并分析两组治疗前后心功能[左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左室射血分数(LVEF)]及血浆脑钠肽(BNP)、基质金属蛋白酶-9(MMP-9)、金属蛋白酶组织抑制因子-1(TIMP-1)水平。结果观察组总有效率(97.30%)较常规组(78.38%)高(P<0.05)。治疗后,两组LVEF升高,LVESD、LVEDD缩小,且观察组LVEF较常规组高,LVESD、LVEDD较常规组小(P<0.05)。治疗后,两组血浆MMP-9、BNP水平下降,TIMP-1水平升高,且观察组血浆MMP-9、BNP水平较常规组低,TIMP-1水平较常规组高(P<0.05)。结论防己黄芪汤合真武汤加减治疗气虚血瘀型CHF患者,可在一定程度上改善心室重构,提升心功能。
Objective To analyze the effects of addition and reduction of Fangji Huangqi Decoction and Zhenwu Decoction in patients with chronic heart failure(CHF)of qi deficiency and blood stasis.Methods A total of 74 patients with CHF of Deficiency of Qi and Blood Stasis who were treated in Xiangfu District Chinese Medicine Hospital from February 2018 to January 2019 were selected,and they were divided into observation group(37 cases)and routine group(37 cases)according to the treatment method.The conventional group accepted furosemide tablets,enalapril maleate tablets and digoxin tablets,and the observation group accepted addition and reduction of Fangji Huangqi Decoction and Zhenwu Decoction on the basis of the conventional group.The curative effects of the two groups were evaluated,and the cardiac function[left ventricular end systolic dimension(LVESD),left ventricular end diastolic dimension(LVEDD),left ventricular ejection fraction(LVEF)],the levels of brain natriuretic peptide(BNP),matrix metalloproteinase-9(MMP-9)and tissue inhibitor of metalloproteinase-1(TIMP-1)of the two groups were analyzed before and after treatment.Results The total effective rate of the observation group was higher than that of conventional group(P<0.05).After treatment,the LVEF of the two groups increased,the LVESD and LVEDD of the two groups decreased,and the LVEF of the observation group was higher than that of the conventional group,the LVESD and LVEDD of the observation group were shorter than those of the conventional group(P<0.05).After treatment,the levels of MMP-9 and BNP of the two groups decreased,the levels of TIMP-1 of the two groups increased,and the levels of MMP-9 and BNP of the observation group were lower than those of the conventional group,the level of TIMP-1 of the observation group was higher than that of the conventional group(P<0.05).Conclusion Addition and reduction of Fangji Huangqi Decoction and Zhenwu Decoction can improve the ventricular remodeling of patients with CHF of Deficiency of Qi and Blood Stasis and enhance cardiac function.
