摘要
目的比较超微经皮肾镜与输尿管软镜治疗肾结石的疗效和安全性。方法抽取济南市中心医院2019年1月至2019年12月收治的肾结石患者173例,回顾性分析其临床资料。根据治疗方式将患者分为输尿管软镜组(102例)和超微经皮肾镜组(71例)。比较两组手术时间、术中出血量、术后住院时间、疗效、术后结石清除率和并发症发生情况。结果输尿管软镜组手术时间、术后住院时间均长于超微经皮肾镜组,术中出血量多于超微经皮肾镜组(P<0.05)。输尿管软镜组总有效率为78.4%(80/102),低于超微经皮肾镜组的97.2%(69/71),P<0.05。输尿管软镜组单次治疗结石清除率为75.5%(77/102),低于超微经皮肾镜组的94.4%(67/71),P<0.05。超微经皮肾镜组并发症发生率为4.2%(3/71),低于输尿管软镜组的13.7%(14/102),P<0.05。结论超微经皮肾镜治疗肾结石可缩短手术时间和术后住院时间,减少术中出血量,提高疗效和结石清除率,降低术后感染等并发症发生率,安全性好。
Objective To compare and analyze the efficacy and safety of ultramicro percutaneous nephroscopy versus flexible ureteroscopy in the treatment of renal calculi.Methods A total of 173 patients with renal calculi admitted to Jinan Central Hospital from January 2019 to December 2019 were selected,and their clinical data were retrospectively analyzed.According to treatment methods,they were divided into flexible ureteroscopy group(102 cases)and ultramicro percutaneous nephroscopy group(71 cases).The operation time,intraoperative blood loss,postoperative hospital stay,curative effect,postoperative stone clearance rate and complications were compared between the two groups.Results The operation time and postoperative hospital stay in flexible ureteroscopy group were significantly longer than those in ultramicro percutaneous nephroscopy group,and the amount of bleeding during operation was more than that in ultramicro percutaneous nephroscopy group(P<0.05).The total effective rate of flexible ureteroscopy group was 78.4%(80/102),lower than the 97.2%(69/71)of ultramicro percutaneous nephroscopy group(P<0.05).The stone clearance rate of flexible ureteroscopy group was 75.5%(77/102),lower than the 94.4%(67/71)of ultramicro percutaneous nephroscopy group(P<0.05).The incidence of complications in ultramicro percutaneous nephroscopy group was 4.2%(3/71),lower than the 13.7%(14/102)in flexible ureteroscopy group,P<0.05.Conclusions Ultramicro percutaneous nephroscopy in treatment of renal calculi can shorten the operation time and postoperative hospital stay,reduce intraoperative blood loss,improve curative effect and stone clearance rate,and reduce postoperative infection and other complications,which is safe.
