摘要
目的分析重症冠心病患者冠状动脉搭桥(CABG)术后发生静脉桥狭窄的影响因素。方法抽取2018年9月至2020年9月在郑州大学第一附属医院行CABG术后随访1年内发生静脉桥狭窄的46例重症冠心病患者为发生组,另抽取同期经CABG术后随访1年内未发生静脉桥狭窄的46例重症冠心病患者为未发生组,回顾性分析两组患者的临床资料。统计并比较两组患者的一般资料、实验室指标、影像学检查等基线资料,经单因素及多因素分析找出重症冠心病患者CABG术后发生静脉桥狭窄的影响因素。结果两组年龄、合并糖尿病、合并高脂血症、血小板平均体积(MPV)、血小板计数(PLT)、脂蛋白α水平比较差异有统计学意义(P<0.05),组间其他资料比较差异未见统计学意义(P>0.05)。经Logistic回归分析结果显示,年龄大、合并糖尿病、合并高脂血症、MPV过表达、PLT过表达、脂蛋白α过表达均是重症冠心病患者CABG术后发生静脉桥狭窄的影响因素(OR>1,P<0.05)。结论重症冠心病患者CABG术后发生静脉桥狭窄可能受患者年龄大、合并糖尿病、合并高脂血症、MPV过表达、PLT过表达、脂蛋白α过表达等因素影响。
Objective To analyze the influencing factors of vein graft stenosis after coronary artery bypass graft(CABG)in patients with severe coronary heart disease.Methods Forty-six patients with severe coronary heart disease who had vein graft stenosis within 1 year of follow-up after CABG in the First Affiliated Hospital of Zhengzhou University from September 2018 to September 2020 were selected as the occurrence group.In addition,46 patients with severe coronary heart disease who were without vein graft stenosis within 1 year of follow-up after CABG at the same period were selected as the non occurrence group.And the clinical data of the two groups were analyzed retrospectively.The general data,data of laboratory index test results and imaging examination data of the two groups were recorded and compared.Univariate and multivariate analysis were used to find out the influencing factors of vein graft stenosis after CABG in patients with severe coronary heart disease.Results There were significant differences in age,diabetes mellitus,hyperlipidemia,mean platelet volume(MPV),platelet count(PLT)and lipoproteinαbetween the two groups(P<0.05).But there was no significant difference in other data between the two groups(P>0.05).The results of Logistic regression analysis showed that old age,combination with diabetes mellitus,complication with hyperlipidemia,MPV overexpression,PLT overexpression and lipoproteinαoverexpression were the influencing factors of vein graft stenosis after CABG in patients with severe coronary heart disease(OR>1,P<0.05).Conclusions The occurrence of vein graft stenosis in patients with severe coronary heart disease after CABG may be affected by factors such as old age,combination with diabetes mellitus,complication with hyperlipidemia,MPV overexpression,PLT overexpression and lipoproteinαoverexpression.
