探究基于疾病轨迹模式干预对骨外科患者肢体功能及生活质量的影响

摘要 目的探究基于疾病轨迹模式干预对骨外科患者肢体功能及生活质量的影响。方法选取我院自2016年6月至2019年6月收治的76例骨外科患者,采取随机数字表法分为对照组与观察组,每组38例,对照组给予常规干预方法,观察组在对照组的基础上实施基于疾病轨迹模式干预,对比两组患者干预前、干预后3个月Fugl-Meyer运动功能量表(Fugl-Meyer Motor Function Scale,FMA)、日常生活能力评级量表(Barthel指数,Barthel Index)、视觉模拟评分法(Visual Analogue Scale,VAS)、汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评分、社会活动调查量表(Functional Activities Questionnaire,FAQ)评分、生活质量(Quality of Life Scale,QOL-C30)评分。结果两组干预后与干预前相比FMA评分及Barthel指数均升高,VAS评分较低;HAMD及FAQ评分均降低;躯体功能、角色功能、情绪功能、认知功能、社会功能以及疲乏评分均升高,观察组干预后较对照组干预后相比上述评分改善更显著,差异具有统计学意义(P<0.05)。结论将基于疾病轨迹模式干预应用于骨外科患者中,有效促进改善了患者肢体功能及日常生活能力,降低了术后疼痛感,能够缓解负性情绪,提高社会活动度,同时促进改善了患者的生活质量。 Objective To explore the effect of intervention based on disease trajectory on limb function and quality of life in patients with orthopedic surgery.Methods A total of 76 cases of orthopedic patients admitted from June 2016 to June 2019 in our hospital were selected and divided into a control group and an observation group using random number table method.The control group was given conventional intervention methods,the group implemented intervention based on the disease trajectory model on the basis of the control group,comparing the Fugl-Meyer Assessment(FMA),Daily Living Ability Rating Scale(Barthel Index)of the two groups of patients before and 3 months after the intervention,Visual analogue scale(VAS),Hamilton Depression Rating Scale(HAMD)score,Social Activities Questionnaire(FAQ)score,quality of life(European Organization for Research and Treatment of Cancer Quality of Life-Core 30,EORTC QOL-C30)score.Results After intervention,the FMA and Barthel Index scores were higher and the VAS scores were lower after the intervention in both groups;the HAMD and FAQ scores were lower;physical function,role function,emotional function,cognitive function,social function and fatigue score were all increased.After the intervention of the observation group,the improvement of the above score was more significant than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Applying the intervention based on the disease trajectory to patients in orthopedic surgery effectively promotes improved limb function and daily living ability,reduces postoperative pain,can relieve negative emotions,improve social activity,and promote and improve the patient's life quality.
