摘要
银屑病是一种以红斑、丘疹、鳞屑损害为主要表现的慢性、瘙痒性皮肤病,在临床中极为常见。乌蛇驱风汤出自《朱仁康临床经验集》,具有搜风清热、败毒止痒的功效,它是治疗荨麻疹、扁平苔藓、瘙痒症等以瘙痒为主的皮肤病方剂。银屑病(进行期)常表现为湿热蕴结体内,瘙痒剧烈,若在此基础上又感染风热之邪,湿热兼加外风客于肌肤,致使肌表凝结邪气,久聚不散而发病。王玉玺教授认为风邪、血热、毒邪共为银屑病(进行期)的致病因素,并强调风邪的重要性,因此在临床治疗中除了要清热、凉血、解毒以外,还要兼顾祛风之法。在银屑病(进行期)的用药经验上,王老善用虫类药以搜刮内风,又善用虫类药的毒性,以达到以毒攻毒的作用。王教授灵活运用乌蛇驱风汤加减治疗银屑病(进行期)临床疗效颇佳,屡试屡验。该文从病因病机、治疗经验、经典案例等方面对王玉玺教授运用乌蛇驱风汤加减治疗银屑病(进行期)的经验进行探讨。
Psoriasis is a chronic,pruritus skin disease mainly manifested by erythema,papules and scales damage,and is very common in clinical practice.Wushe Qufeng Decoction(乌蛇驱风汤) from ZHU Renkang′s Clinical Experience Collection,has the function of searching wind,clearing heat and anti-pruritic effect.It is the treatment of urticaria,lichen planus,pruritus and other skin diseases based on pruritus prescription.Psoriasis(progressive period) is often manifested as dampness and heat accumulating inside the body.Pruritus is intense.On this basis,if the patient is infected with the evils of wind-heat,damp-heat and external wind,resulting in the condensation of evil Qi on the surface of the muscle,and the disease will occur after a long time.Professor WANG Yuxi thinks that wind evil,blood-heat and toxin are the pathogenic factors of psoriasis(in progress period),and emphasizes the importance of wind evil.Therefore,in addition to clearing away heat,cooling blood and detoxifying,the method of dispelling wind should also be considered in clinical treatment.In the medication experience of psoriasis,professor WANG is good at using insect drugs to dispel internal wind,and good at using the toxicity of insect drugs to achieve the effect of treating toxin with toxic drugs.Professor WANG flexibly uses Wushe Qufeng Decoction in the treatment of psoriasis(in progress period) and the effect is quite good.This paper discussed Professor WANG Yuxi′s experience in the treatment of psoriasis(in progress period) with Wushe Qufeng Decoction from etiology,pathogenesis,treatment experience and classic cases.
