摘要
目的探讨益肾通络法结合针刺治疗脑卒中后肩手综合征(SHS)的临床效果。方法选取江西省景德镇市中医医院中风病科于2018年10月—2020年6月收治的60例脑卒中后SHS患者为研究对象,按照随机数字表法分为对照组(30例)与治疗组(30例)。对照组单纯行针刺治疗,治疗组在对照组的基础上联合益肾通络法治疗。分别于治疗前及治疗10 d时采用视觉模拟量表(VAS)对两组的疼痛情况进行评定,同时采用上肢功能评定表(DASH)评定两组的上肢从事日常生活的能力,使用公分软尺对两组的手肿胀程度进行测定,并比较两组的临床疗效。结果两组护理前的DASH、肿胀及VAS评分比较,差异无统计学意义(P>0.05);治疗10 d时,治疗组的DASH、肿胀及VAS评分均低于对照组,差异有统计学意义(P<0.05)。治疗组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论针对脑卒中后SHS患者,采用益肾通络法与针刺相联合方案的治疗效果较好,能提高患肢日常生活能力,改善肿胀、肩痛症状,临床应用价值较高。
Objective To explore the clinical efficacy of Yishen Tongluo method combined with acupuncture in the treatment of shoulder hand syndrome(SHS)after stroke.Methods A total of 60 post-stroke SHS patients who were admitted to the Department of Stroke Diseases of Jingdezhen Hospital of Traditional Chinese Medicine in Jiangxi Province from October 2018 to June 2020 were selected as the research subjects,and they were divided into the control group(30 cases)and the treatment group(30 cases)according to the random number table method.The control group received only acupuncture treatment,the treatment group was given Yishen Tongluo tmethod on the basis of the control group.The visual analog scale(VAS)was used to evaluate the pain of the two groups before treatment and 10 days after treatment.The upper limb function assessment scale(DASH)was used to assess the ability of the upper limbs to engage in daily life in the two groups,and the degree of hand swelling in the two groups was measured with a centimeter soft ruler,and the clinical effects were compared between the two groups.Results There was no significant difference in the scores of DASH,swelling and VAS between the two groups before nursing(P>0.05).At 10 days of treatment,the scores of DASH,swelling and VAS in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with post-stroke SHS,the treatment effect of Yishen Tongluo method combined with acupuncture is better,which can improve the ability of daily life of the affected limb,improve the symptoms of swelling and shoulder pain,and has high clinical application value.
