不同咬合板治疗颞下颌关节紊乱病对患者上气道影响的CBCT研究 被引量:4

The changes of upper airway CBCT images of temporomandibular disorders patients after different types of splints treatments

摘要 目的:分析稳定性咬合板(stabilization splint,SS)及前导性再定位咬合板(anterior repositioning splint,ARS)治疗前后颞下颌关节紊乱病(temporomandibular disorders,TMDs)患者上气道变化。方法:选择56例厦门医学院附属口腔医院就诊的双侧髁突后下移位的TMDs患者,根据不同咬合板治疗方式分为SS组及ARS组(每组28例)。治疗前后均拍摄CBCT,Dolphin Image 11.9软件重建三维图像。测量咬合板治疗前后患者鼻咽腔容积、口咽腔容积、口鼻咽腔总容积及口咽腔最小横截面积的变化。结果:与治疗前相比,SS治疗后鼻咽腔容积增大(P<0.05);ARS治疗后患者口咽腔容积、口鼻咽腔总容积显著增大(P<0.05)。结论:两种咬合板治疗TMDs均有助于患者通气,ARS与SS相比更有助于增加患者咽腔通气量,治疗上气道狭窄TMDs患者更具优势。 Objective:To investigate the effects of stabilization splint(SS)and anterior repositioning splint(ARS)on the changes of upper airway in patients with temporomandibular disorders(TMDs)before and after treatment.Methods:Fiftysix TMDs patients with bilateral condyles moved posteroinferiorly were selected.According to different occlusal splints treatment methods,all the patients were divided into two groups(SS group and ARS group,28 cases in each group),whose CBCT images before and after treatment were reconstructed by using Dolphin Image 11.9 software.The nasopharyngeal volume,oropharyngeal volume,total pharyngeal volume and minimum oropharyngeal cross-sectional area were measured before and after the treatment,and the changes were analyzed.Results:The nasopharyngeal volume was significantly increased after SS treatment,and the oropharyngeal volume and total pharyngeal volume were increased significantly after ARS treatment.Conclusion:Two types of splints are all helpful for ventilation of TMDs patients,and ARS is more advantageous for pharyngeal volume enlargement which is more suitable for TMDs patients with upper airway stenosis.
