衡水市2016-2018年医疗照射水平调查研究

Investigation on medical exposure level in Hengshui from 2016 to 2018

摘要 目的了解衡水市医疗机构的医疗照射现状,为放射诊疗设备配置管理和放射诊疗的监管提供科学依据,促进放射诊疗的合理发展。方法选取衡水市10家开展放射诊疗的医疗机构作为调查对象,采用全国统一的调查表格进行调查,对所收集的数据进行整理和统计分析。结果衡水市2016-2018年10家医疗机构放射诊疗设备和放射工作人员总数均不断增加,但使用每台设备的放射工作人员数无明显变化。各级别医疗机构3年间医疗照射总人次平均增长速度为6.33%,医疗照射频次平均增长速度为8.33%,其中一级医疗机构的医疗照射总人次上升速度最快,平均增长速度为14.52%。放射诊疗项目中,医用X射线诊断照射人次占比例最大;介入放射学照射人次和应用频次增长最快,分别为22.19%和23.59%;放射诊疗近3年呈下降状态。二级医疗机构的医疗照射总人次、医疗照射频次和所占医疗机构诊疗总人次的百分比均为最高。各级别医疗机构放射诊疗项目频次差异有统计学意义(χ^(2)=617 184.5,P<0.05)。各级别医疗机构医用X射线诊断、介入放射学和放射诊疗照射频次差异均有统计学意义(χ^(2)=26 532.5、1 050.4、475.1,P<0.05)。结论各级别医疗机构和不同放射诊疗项目频次有差异,衡水市医疗照射水平呈逐年上升趋势,因此需控制放射诊疗工作人员和公众的受照剂量,确保受检者和患者辐射的合理性,放射诊疗工作的正当化。 Objective To understand current situation of medical exposure in Hengshui medical institutions,to provide scientific basis for configuration management of radiological diagnosis and radiotherapy equipment and supervision on radiological diagnosis and treatment,and to promote rational development of radiological diagnosis and treatment.Methods Ten medical institutions in Hengshui were selected as respondents,unified national survey form was used for investigation,and the collected data were sorted out and statistically analyzed.Results The total number of radiological diagnosis and treatment equipment and radiologic worker in 10 medical institutions in Hengshui increased from 2016 to 2018,but the number of radiologic workers using all the equipment did not change significantly.The growth rate of medical exposure in medical institutions at all levels within three years is 6.33%,and the growth rate of medical exposure frequency was 8.33%on average.Among them,the rate of increase in the total number of total number of medical irradiation in first-level medical institutions was the fastest,and mean growth rate hit 14.52%.In radiologic diagnosis and radiotherapy items,the proportion of medical X ray diagnostic irradiation was the top,the person-time of interventional radiology exposure and frequency of application increased fastest,hitting 22.19%and 23.59%respectively,and the radiological diagnosis and treatment decreased within the last three years.The total person-time of medical exposure,frequency of medical exposure and percentage of total number of medical treatment in secondary medical institutions were the top.The difference in the frequency of radiologic diagnosis and treatment items in different medical institutions was statistically significant(χ^(2)=617184.5,P<0.05).The difference in the radiation exposure frequency of medical X diagnosis,interventional radiology and radiological diagnosis and treatment was statistically significant(χ^(2)=26532.5,1050.4,475.1,P<0.05).Conclusion The frequency of different levels of medical institutions and different radiological diagnosis and treatment items varies,and the level of medical exposure in Hengshui is increasing year by year.Hence,it is necessary to control the exposure dose of radiological diagnosis and treatment workers and the public to ensure the rationality of radiation on subjects and patients and proper proportion of radiological diagnosis and treatment services.
