儿童新型甲型流感疫苗接种后远期T淋巴细胞介导的免疫功能变化研究 被引量:1

Long-term T lymphocyte-mediated changes in immune function after immunization of children with new influenza A H1N1 vaccine

摘要 目的探讨儿童新型甲型H1N1流感疫苗接种后远期T淋巴细胞介导的免疫功能变化。方法选取2017年6月至2019年9月于该院进行新型甲型H1N1流感疫苗接种的儿童80例作为研究对象,将经检测有甲型H1N1流感抗体的儿童作为观察组(40例),无甲型H1N1流感抗体的儿童作为对照组(40例)。采用酶联免疫吸附试验(ELISA)与流式微球捕获芯片技术(CBA)测定儿童免疫功能变化及T淋巴细胞介导的免疫情况,随访并测定接种疫苗的儿童T淋巴细胞亚群变化情况。结果疫苗接种后,观察组中干扰素(IFN)-γ、肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-4、IL-10水平较对照组显著降低,差异均有统计学意义(P<0.05);随访结果显示,观察组T淋巴细胞CD3+、CD4+、CD8+,以及IgG、IgM、IgA水平较对照组显著降低,差异均有统计学意义(P<0.05);观察组疫苗接种后不良反应(局部疼痛、局部瘙痒、局部红肿、头痛头晕、发热、恶心呕吐)发生率明显低于对照组,差异有统计学意义(P<0.05)。结论儿童新型甲型H1N1流感疫苗接种后再刺激T淋巴细胞亚群介导的免疫功能变化,对儿童早期风险评估有重要的预防参考价值,且安全性较高。 Objective To investigate the changes of long-term T lymphocyte mediated immune function in children with new influenza A H1N1 vaccine.Methods From June 2017 to September 2019,a total of 80 children who received the new influenza A vaccine in the hospital were selected as the study subjects.Forty children who had H1N1 influenza antibodies were selected as the observation group,and forty children who didn′t had H1N1 influenza antibodies were selected as the control group.ELISA and CBA methods were used to determine the changes in children′s immune function and T lymphocyte-mediated immunity,and the changes of T lymphocyte subsets in vaccinated children were measured during follow-up period.Results After vaccination,the levels of IFN-γ,TNF-α,IL-4 and IL-10 in the observation group were significantly decreased compared with the control group,with statistical significance(P<0.05).Follow-up results showed that T lymphocyte CD3+,CD4+,CD8+,IgG,IgM and IgA in the observation group were significantly lower than those in the control group,with statistical significance(P<0.05).The incidence of adverse reactions(local pain,local pruritus,local redness,headache,dizziness,fever,nausea and vomiting)in the observation group after vaccination was significantly lower than that in the control group,with statistical significance(P<0.05).Conclusion The changes of immune function mediated by T lymphocyte subsets after the inoculation of the new influenza A H1N1 vaccine in children have important preventive reference value for the early risk assessment of children,and the safety is high.
