Effect of femoral nerve block on analgesia following hip arthroplasty

摘要 [目的]探索股神经阻滞对全髋关节置换术(total hip arthroplasty,THA)后镇痛的影响。[方法]2018年1月—2020年1月进行的114例择期THA术的患者分为两组:全麻组61例,全麻+神经阻滞组(联合组)53例。比较两组术中资料与术后早期镇痛资料。[结果]两组患者均顺利手术,术中麻醉平稳,无严重不良反应和术中并发症。两组患者在清醒时间、手术时间、丙泊酚及瑞芬太尼总用量差异无统计学意义(P>0.05)。术后60 min、120 min和24 h,联合组VAS评分显著低于全麻组(P<0.05);术后60 min、120 min、24 h、48 h及72 h,联合组镇痛泵按压次数显著低于全麻组(P<0.05)。[结论]术中全麻辅以股神经阻滞可显著降低患者术后急性疼痛程度并显著降低术后阿片类药物的使用。 [Objective]To explore the effect of femoral nerve block on analgesia after total hip arthroplasty(THA).[Methods]A total of 114 patients who were undergoing selective THA from January 2018 to January 2020 were divided into two groups.Of them,61 patients received THA by general anesthesia(the GA group),while the other 53 patients had THA performed with general anesthesia combined with nerve block(the combined group).The intraoperative data and the early postoperative analgesia data were compared between the two groups.[Results]Patients in both groups were operated on smoothly with stable intraoperative anesthesia,whereas with no serious adverse reactions and intraoperative complications.There was no significant difference in the awaking time,operation time,total dosage of propofol and remifentanil between the two groups of patients(P>0.05).At 60 min,120 min and 24 hours after operation,the VAS scores of the combined group were significantly lower than those of the GA group(P<0.05).At 60 min,120 min,24 hours,48 hours and 72 hours after operation,the frequency of analgesic pump compressions in the combined group was significantly lower than that of the GA group(P<0.05).[Conclusion]Intraoperative general anesthesia supplemented with femoral nerve block does significantly reduce the degree of acute postoperative pain,in addition,reduce the use of opioids after surgery.
