摘要
目的:观察无创呼吸机联合尼可刹米治疗AECOPD合并Ⅱ型呼吸衰竭高碳酸血症的临床效果。方法:60例AECOPD合并Ⅱ型呼吸衰竭高碳酸血症随机分为对照组与观察组,对照组接受无创呼吸机治疗,观察组在对照组的基础上加用尼可刹米治疗。比较两组的临床治疗效果,治疗后血气分析指标、肺功能指标及炎症因子改善情况。结果:观察组总有效率为96.67%,高于对照组的总有效率73.33%。治疗后,观察组PaCO2低于对照组,PaO2高于对照组。治疗后观察组FEV1、FVC、PEF均高于对照组。治疗后观察组PCT、CRP、sTREM-1、HMGB-1水平均低于对照组。结论:对AECOPD合并Ⅱ型呼吸衰竭高碳酸血症患者,应用无创呼吸机联合尼可刹米治疗,可提高临床治疗效果,改善血气分析指标、肺功能指标,减轻炎症因子水平。
Objective To observe the clinical effect of non-invasive ventilator combined with Nicosamide in the treatment of AECOPD combined with type Ⅱ respiratory failure hypercapnia.Methods 60 cases of AECOPD combined with type II respiratory failure hypercapnia were randomly divided into the control group and the observation group.The control group received non-invasive ventilator treatment,and the observation group was treated with nicoximide on the basis of the control group.The clinical treatment effects of the two groups were compared,and the improvement of blood gas analysis indexes,lung function indexes and inflammatory factors after treatment.Results The total effective rate of the observation group was 96.67%,which was higher than the total effective rate of the control group of 73.33%.After treatment,PaCO2 of the observation group was lower than that of the control group,and PaO2 was higher than that of the control group.After treatment,FEV1,FVC and PEF in the observation group were higher than those in the control group.After treatment,the levels of PCT,CRP,sTREM-1 and HMGB-1 in the observation group were lower than those in the control group.Conclusion For patients with AECOPD and type II respiratory failure hypercapnia,the application of non-invasive ventilator combined with Nicosamide can improve the clinical treatment effect,improve blood gas analysis indicators,lung function indicators,and reduce the level of inflammatory factors.
