摘要
目的探讨经双源CT双下肢CTA检查的下肢腘动脉延续支的解剖特点、变异情况及临床分型。方法回顾性分析2015年4月-2019年10月行双源CT双下肢CTA检查的217例(239肢)患者的影像学资料。总结分析腘动脉延续支的特点及变异情况,并进行临床分型,统计发生率;并对临床常见ⅠA型,测量腓骨小头近端水平面至第1延续支起点距离(左侧C、右侧C)、至第2延续支起点距离(左侧D、右侧D)。结果双侧下肢CTA显影清晰99例,仅左侧下肢CTA显影清晰13例,仅右侧下肢CTA显影清晰28例;双侧均显影不清77例。双下肢CTA检查影像能准确显露双下肢腘动脉延续支情况,根据延续支特点及变异情况,将腘动脉延续支分为Ⅰ型(ⅠA、ⅠB)、Ⅱ型(ⅡA、ⅡB)、Ⅲ型(ⅢA、ⅢB)。其中ⅠA型为临床常见类型,发生率为90.37%;其次为ⅠB型,发生率为5.44%;Ⅱ型(ⅡA、ⅡB)发生率2.93%;Ⅲ型(ⅢA、ⅢB)临床发生率较低,为1.25%。对ⅠA型,测量腓骨小头近端水平面至第1延续支起点距离:左侧C(34.971±5.287)mm、右侧C(37.003±5.260)mm;至第2延续支起点距离:左侧D(60.366±9.357)mm、右侧D(64.256±11.582)mm。结论双下肢腘动脉延续支基于双下肢CTA分型及延续支距离测量,能准确显示腘动脉延续支情况、起点距离及变异情况,为临床医生提供一种简便可靠的方法;对腘动脉损伤的详细诊断及手术治疗,具有良好的临床指导意义。
Objective To investigate the characteristics,variability and classification of the continued branch of the popliteal artery in the lower extremities by CTA examination using dual-source CT.Methods Clinical retrospective analysis of the CT images of double-lower limb CTA was performed in our hospital from April 2015 to October 2019.A total of 217 cases of 434 limbs were included.There were 158 males and 59 females aged from 18 to 89 years(average age 53.5 years)We summarized and analyzed the characteristics and variation of the continued branch of the popliteal artery and then divided into types,For the type IA,we measured the distance from the proximal fibula head horizontal level to the origin of the first continuation branch(left C,right C),and the distance to the origin of the second continuation branch(left D,right D).Results Clearly developed limbs included 99 bilateral limbs,13 limbs on the left and 28 limbs on the right.The CTA examination images of the lower limbs could accurately reveal the continuation branches of the iliac arteries of the lower limbs.According to the characteristics and variation of the continuation branches,the continuation branches of the popliteal artery were divided into typeⅠ(ⅠA,ⅠB),typeⅡ(ⅡA,ⅡB),and typeⅢ(ⅢA,ⅢB),of whichⅠA was a common clinical type with an incidence rate of 90.37%;followed by typeⅠB with an incidence rate of 5.44%;typeⅡ(ⅡA,ⅡB)incidence rate was 2.93%,and typeⅢ(ⅢA,ⅢB)had low rate,which was 1.25%.For typeⅠA,We measured the distance from the proximal end of the fibula small head to the starting point of the first continuation branch:left C(34.971±5.287)mm,right C(37.003±5.260)mm;to the second distance from the starting point of extension:D on the left(60.366±9.357)mm,and D on the right(64.256±11.582)mm.Conclusion Based on the CTA classification of the lower limbs and the measurement of the distance between the branches,we can accurately understand the condition of the branches of the popliteal artery,the starting distance and the variation.It provides a simple and reliable method for clinician and it has a good clinical diagnosis and surgical treatment of the popliteal artery injury.It has good clinical guiding significance for the treatment of diseases.
