摘要
目的分析硬膜外复合静脉全身麻醉(全麻)在老年食管癌根治术患者中的应用价值。方法40例老年食管癌患者,根据手术麻醉方式不同分为对照组与观察组,各20例。对照组患者实施全麻,观察组在对照组基础上结合硬膜外麻醉支持。比较两组患者平均动脉压(MAP)、心率(HR)水平,麻醉药物用量、苏醒时间、拔管时间及不良事件发生情况。结果插管即刻与拔管即刻,观察组MAP及HR水平均低于对照组,差异具有统计学意义(P<0.05)。观察组瑞芬太尼及丙泊酚用量分别为(0.3±0.1)、(176.3±4.2)mg,均低于对照组的(0.5±0.2)、(286.0±4.8)mg,差异具有统计学意义(P<0.05);观察组苏醒时间及拔管时间均短于对照组,差异具有统计学意义(P<0.05)。观察组麻醉不良事件发生率10.0%低于对照组的40.0%,差异具有统计学意义(P<0.05)。结论采用硬膜外麻醉复合静脉全麻,可保证老年食管癌根治术治疗患者生命体征稳定,减少药物用量,避免应激反应,预后好。
Objective To analyze the practical value of epidural anesthesia combined intravenous general anesthesia in elderly patients undergoing radical resection of esophageal cancer.Methods A total of 40 elderly patients with esophageal cancer were divided into control group and observation group according to different anesthesia methods,with 20 cases in each group.The control group received general anesthesia,and the observation group received epidural anesthesia based on the control group.The mean arterial pressure(MAP),heart rate(HR),dosage of anesthetic drugs,wake-up time,extubation time,and occurrence of adverse events were compared between the two groups.Results Immediately after intubation and extubation,the MAP and HR of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).The dosage of remifentanil and propofol of the observation group were(0.3±0.1)and(176.3±4.2)mg,which were lower than(0.5±0.2)and(286.0±4.8)mg of the control group,and the difference was statistically significant(P<0.05).The wake-up time and extubation time of the observation group were shorter than those of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse events of the observation group was 10.0%,which was lower than 40.0%of the control group,and the difference was statistically significant(P<0.05).Conclusion Epidural anesthesia combined intravenous general anesthesia can ensure the stability of vital signs,reduce the dosage of drugs,avoid stress response and have a good prognosis in elderly patients undergoing radical resection of esophageal cancer.
