摘要
目的探讨女性HIV感染者/AIDS患者(HIV/AIDS)高脂血症患病率及其影响因素,为临床提前干预提供依据。方法以"艾滋病综合防治数据信息系统"和"中医药治疗艾滋病数据库"为数据来源,收集河南中医项目地区2018年女性HIV/AIDS患者相关信息,利用SPSS软件,采用Logistic回归方法分析女性HIV/AIDS患者高脂血症的影响因素。结果共纳入1538例女性HIV/AIDS患者,平均年龄(49.36±8.84)岁,其中高脂血症414例(26.92%),HIV确诊时间>10年(OR=0.363,95%CI:0.174~0.756)、含LPV/r HAART方案(OR=1.522,95%CI:1.161~1.997)、HAART治疗>10年(OR=3.139,95%CI:1.548~6.367)、肝功能损伤(OR=1.676,95%CI:1.196~2.35)、血糖升高(OR=2.075,95%CI:1.507~2.856)、CD4^(+)细胞计数350~500个/mL(OR=2.27,95%CI:1.223~4.212)、CD4^(+)细胞计数>500个/mL(OR=3.139,95%CI:1.727~5.704)、服用中药(OR=1.463,95%CI:1.086~1.972)均是高脂血症的危险因素。结论HIV感染病程的延长、HAART服药时间的延长、含LPV/r HAART方案、肝损伤、血糖升高、CD4^(+)细胞计数升高、服用中药均是高脂血症的危险因素,其中CD4^(+)细胞计数与高脂血症呈正相关,应提前进行预防干预。
Objective To investigate the prevalence of hyperlipidemia in HIV/AIDS patients and its predisposing factors,so as to provide basis for clinical intervention in advance.Methods Based on the data from"AIDS comprehensive prevention and treatment data information system"and"traditional Chinese medicine treatment of AIDS database",the related information of female HIV/AIDS patients in Henan TCM project area in 2018 was collected,and the predisosing factors of hyperlipidemia in female HIV/AIDS patients were analyzed by means of SPSS software and Logistic regression method.Results A total of 1538 female HIV/AIDS patients were enrolled,with an average age of(49.36±8.84)years old.Among them,414(26.92%)were hyperlipidemia.The course since HIV positive>10 years(OR=0.363,95%CI:0.174~0.756),treatment with LPV/r HAART(OR=1.522,95%CI:1.161~1.997),the course since taken HAART>10 years(OR=3.139,95%CI:1.548~6.367),liver function injury(OR=1.676,95%CI:1.196~2.35),blood glucose(OR=2.075,95%CI:1.507~2.856),CD4^(+) cell count 350~500 cells/mL(OR=2.27,95%CI:1.223~4.212),CD4^(+)cell count>500 cells/mL(OR=3.139,95%CI:1.727~5.704)and taking Chinese medicine(OR=1.463,95%CI:1.086~1.972)are all risk factors for hyperlipidemia.Conclusion The predisposing factors of hyperlipidemia include the prolongation of HIV infection course and HAART medication time,LPV/r HAART regimen,liver injury,blood glucose,CD4 count and taking Chinese medicine,and there was a positive correlation between CD4 count and hyperlipidemia,which should be prevented and intervened in advance.There was a positive correlation between CD4 count and hyperlipidemia,Chinese medicine in HIV/AIDS female patients.
