摘要
目的探讨彩色多普勒超声心动图在小儿先天性心脏病(先心病)诊断中的应用价值。方法方便选择2017年3月—2020年2月72例先心病患儿为观察组,超声心动图检查未发现先心病的正常儿童27名为对照组,对两组超声心动图检查进行分析。结果观察组ESV、EDV分别为(15.39±3.27)、(34.22±5.17)mL,高于对照组(12.55±2.44)、(31.78±5.44)mL,差异有统计学意义(t=4.562、2.062,P<0.05);观察组SV、LVEF分别为(18.63±3.21)mL、(51.67±6.49)%,低于对照组(24.34±4.15)mL、(68.81±7.98)%,差异有统计学意义(t=7.256,10.974,P<0.05)。超声心动图诊断小儿先心病的灵敏度为94.44%、特异度为96.30%、诊断符合率为94.95%,其中诊断肺动脉口狭窄、法洛四联症、室间隔缺损、房间隔缺损、动脉导管未闭、心内膜垫缺损的敏感性较高,分别为100.00%、100.00%、94.74%、94.12%、94.12%、87.50%,不同类型先心病的敏感性比较差异无统计学意义(P>0.05)。结论彩色多普勒超声心动图诊断小儿先天性心脏病的敏感性和准确性均较高。
Objective To explore the application value of color Doppler echocardiography in the diagnosis of congenital heart disease(congenital heart disease)in children.Methods From March 2017 to February 2020,72 children with congenital heart disease were selected as the observation group,and 27 normal children with no congenital heart disease found in echocardiography were selected as the control group.The two groups were analyzed by echocardiography.Results The ESV and EDV of the observation group were(15.39±3.27)mL and(34.22±5.17)mL,respectively,which were higher than those of the control group(12.55±2.44)mL and(31.78±5.44)mL.The difference was statistically significant(t=4.562,2.062,P<0.05);SV and LVEF in the observation group were(18.63±3.21)mL and(51.67±6.49)%,respectively,which were lower than those in the control group(24.34±4.15)mL and(68.81±7.98)%,the difference was statistically significant(t=7.256,10.974,P<0.05).The sensitivity of echocardiography in the diagnosis of pediatric congenital heart disease was 94.44%,the specificity was 96.30%,and the diagnostic coincidence rate was 94.95%.Among them,the diagnosis of pulmonary artery stenosis,tetralogy of Fallot,ventricular septal defect,atrial septal defect,patent ductus arteriosus The sensitivity of endocardial cushion defects was high,100.00%,100.00%,94.74%,94.12%,94.12%,87.50%,respectively.The sensitivity of different types of congenital heart disease was not significantly different(P>0.05).Conclusion Color Doppler echocardiography has high sensitivity and accuracy in diagnosing congenital heart disease in children.
