摘要
目的探讨联合检测类风湿因子IgM-RF、IgG-RF、IgA-RF及抗环瓜氨酸肽(CCP)抗体对早期诊断类风湿性关节炎的价值。方法采用整群抽样法抽选2019年10月至2020年10月本院收治的类风湿性关节炎患者设为病例组(n=80),另选取同时间段于我院进行健康体检者为对照组(n=80),统计并对比两组受试者血清中的类风湿因子IgM-RF、IgG-RF、IgA-RF及CCP抗体抗体阳性率情况。结果病例组抗CCP抗体、类风湿因子检测阳性率均高于健康对照组(P<0.05);而病程≤3个月分别与病程≤12个月和病程≤24个月抗CCP抗体、类风湿因子检测阳性率比较差异无统计学意义(P>0.05);IgM-RF、IgG-RF、IgA-RF及抗CCP抗体联合检测对类风湿关节炎的诊断灵敏度、特异度高于类风湿因子的单项检测(P<0.05);特异度与抗CCP抗体单项检测相比差异无意义(P>0.05)。结论联合检测类风湿因子IgM-RF、IgG-RF、IgA-RF及抗环瓜氨酸肽抗体在早期诊断类风湿性关节炎中具有重要帮助,对类风湿性关节炎早期防治意义重大。
Objective To study the value combined examination of rheumatoid factors(RF)IgM-RF,IgG-RF,IgA-RF and anti-cyclic peptide containing citrulline(CCP)antibody on the early diagnosis of rheumatoid arthritis(RA).Methods Through cluster sampling method,80 RA patients from October in to October in 2020 were set as case group.Over the same time period,80 healthy checkups were set as control group.Serum IgM-RF,IgG-RF,IgA-RF and CCP antibody positive rates were counted.Results The positive detection rates of anti-CCP antibody and RF in case group were significantly higher than control group and there were statistical significantly differences(P<0.05).There were no statistical significantly differences about the positive detection rates of anti-CCP antibody and RF among patients with disease courses≤3 m,≤12m and≤24m(P>0.05).The diagnosis sensitivity and specificity based on IgM-RF,IgG-RF,IgA-RF and anti-CCP antibody combined examinations were significantly higher than those based on the single examination(P<0.05).The diagnosis specificity based on combined examinations and anti-CCP antibody there were no statistical significantly differences(P>0.05).Conclusion The combined examinations of IgM-RF,IgG-RF,IgA-RF and anti-CCP antibody is clinically proven to be effective for early diagnosis of RA,providing basis on the early control and treatment of RA.It is worthy of clinical promotion and application.
