窄带成像技术联合染色内镜对消化道早期癌及癌前病变诊断的效果 被引量:4

Effect of narrow-band imaging combined with chromoendoscopy in the diagnosis of early cancer and precancerous lesions of digestive tract

摘要 目的研究消化道早期癌及癌前病变患者行窄带成像技术(NBI)+染色内镜诊断的价值。方法数据采集本院2019年5月至2020年10月收治的100例消化道早期癌及癌前病变患者,根据检查方式分为参照组(普通白光内镜检查,n=50)、试验组(NBI+染色内镜检查,n=50)。比较两组诊断结果、病变轮廓清晰度、疾病检出率。结果以病理学结果为对照,试验组阳性率22.00%,阴性率70.00%;参照组阳性率12.00%,阴性率48.00%,试验组假阳性率为6.00%,假阴性率为2.00%,低于参照组的24.00%及14.00%,与参照组比较,试验组灵敏度、特异度更高;试验组2分分布区域占比为2.00%,低于参照组的24.00%;试验组≤中度非典型增生检出率为20.00%,高于参照组的4.00%,差异有统计学意义(P<0.05)。结论NBI+染色内镜检查能提高消化道早期癌及癌前病变的确诊率、改善患者预后。 Objective To study the diagnostic value of narrow-band imaging(NBI)combined with chromoendoscopy in patients with early cancer and precancerous lesions of digestive tract.Methods The data were collected from 100 patients with early cancer and precancerous lesions of digestive tract admitted to our hospital from May 2019 to October 2020.They were divided into the reference group(common white light endoscopy,n=50)and the experimental group(NBI and chromoendoscopy,n=50)according to the difference of inspection methods.The diagnosis results,the definition of lesion contour and the disease detection rate of the two groups were compared.Results The pathological results were considered as the reference.The positive rate and negative rate of the experimental group were 22.00%and 70.00%,respectively.The positive rate and negative rate of the reference group were 12.00%and 48.00%.The false positive rate and false negative rate were lower in the experimental group(6.00%and 2.00%)than those in the reference group(24.00%and 14.00%).Compared with the reference group,the sensitivity and specificity were higher in the experimental group.The proportion of 2-point distribution area was lower in the experimental group(2.00%)than that in the reference group(24.00%).The detection rate of moderate atypical hyperplasia was higher in the experimental group(20.00%)than that in the control group(4.00%),the difference was statistically significant(P<0.05).Conclusion NBI combined with chromoendoscopy can improve the diagnosis rate and prognosis of early cancer and precancero us lesions patients.
