光学相干断层成像指导下冠状动脉内植入生物可吸收支架的初步经验 被引量:1

Bioabsorbable Vascular Stents Implanted in Coronary Artery with the Guidance of Optical Coherence Tomography:Initial Experience

摘要 目的总结光学相干断层成像(OCT)指导下冠状动脉内植入生物可吸收支架(BRS)的初步经验。方法选取2019年8—12月在泰达国际心血管病医院心内五科行经皮冠状动脉介入治疗(PCI)植入乐普医疗NeoVas支架的11例患者作为研究对象,回顾性分析其临床基线资料,并记录和分析手术数据、OCT数据及随访结果。结果11例患者的平均年龄为(53.4±10.7)岁;男10例,女1例;PCI靶血管为左前降支5例,右冠状动脉4例,左回旋支2例;美国心脏协会/美国心脏病学会(AHA/ACC)分型A型4例,B1型7例;共植入11枚BRS,支架平均直径3.34 mm,平均长度22.6 mm;预扩张球囊、支架、后扩张球囊直径比为0.88∶1.00∶1.08;术中1例支架脱载原位释放,无夹层、血栓、穿孔等并发症;中位随访时间4.8个月,未发生支架内血栓等不良心血管事件。结论在OCT指导下,BRS可安全地被应用于简单病变,但支架通过性欠佳,需警惕脱载风险。 Objective The initial experience of bioabsorbable vascular stents(BRS)implanted in coronary artery with the guidance of optical coherence tomography(OCT)was summarized.Methods 11 patients who had been implanted the NeoVas stents from LEPU MEDICAL by percutaneous coronary intervention(PCI)in the Fifth Department of Cardiology,TEDA International Cardiovascular Hospital were selected as subjects in this study.The clinical baseline data were retrospectively analyzed,and the data of operations and OCT and the follow-up information were recorded and analyzed.Results The average age of 11 patients was 53.4±10.7 years;10 were males,1 was female;PCI target vessels were 5 left anterior descending,4 right coronary artery,2 left circumflex artery;4 cases belonged to American Heart Association/American College of Cardiology(AHA/ACC)type A,and 7 cases belonged to type B1.A total of 11 BRS were implanted and the average diameter and length were 3.34 mm and 22.6 mm,respectively.The diameter ratio of the pre-dilatation balloon,stent,and post-dilatation balloon was 0.88∶1.00∶1.08.One patient had stent loss during operation and no complications occurred such as without interlayer,thrombus and perforation.The median follow-up time was 4.8 months,and no cardiovascular adverse events happened such as stent thrombosis.Conclusion The BRS could be applied to simple pathological changes safely with the guidance of OCT.However,the stent had poor passability and needed to be alert of the risk of stent loss.
