摘要
目的探讨心房颤动(房颤,AF)患者血清转化生长因子-β1(TGF-β1)、网膜素-1(Omentin-1)、半乳糖凝聚素-3(Gal-3)水平变化及与心房纤维化的关系。方法选取2017年3月~2019年7月于濮阳市人民医院心内二科79例经手术治疗的AF患者作为观察组,另选取同期健康体检者79例作为对照组。对比不同心房纤维化程度患者血清TGF-β1、Omentin-1、Gal-3、人Ⅰ型胶原C端肽(PCIP)、Ⅰ型原羟基端交联肽(CITP)水平,分析血清TGF-β1、Omentin-1、Gal-3水平与AF、心房纤维化程度的关系及血清TGF-β1、Omentin-1、Gal-3水平与PCIP、CITP水平相关性。结果观察组血清TGF-β1、Gal-3水平高于对照组,Omentin-1水平低于对照组(P<0.05);高纤维化患者血清TGF-β1、Gal-3、PCIP、CITP水平高于低纤维化患者,Omentin-1水平低于低纤维化患者(P<0.05);AF患者血清TGF-β1、Gal-3、PCIP、CITP水平与心房纤维化程度呈正相关关系(P<0.05),血清TGF-β1、Gal-3为AF发生的危险因素,Omentin-1为保护因素(P<0.05);Omentin-1水平与心房纤维化程度呈负相关关系(P<0.05);AF患者血清TGF-β1、Gal-3与心房纤维化生化标记物PCIP、CITP呈正相关关系(P<0.05);Omentin-1与心房纤维化生化标记物PCIP、CITP呈负相关关系(P<0.05)。结论血清TGF-β1、Omentin-1、Gal-3异常表达可能是心房纤维化、心房结构重塑诱发AF的分子机制,与AF发生、维持有关,可为AF早期诊断、治疗提供参考。
Objective To investigate the changes of serum transforming growth factor-β1(TGF-β1), Omentin-1 and galectin-3(Gal-3) levels in patients with atrial fibrillation(AF) and their relationship with atrial fibrosis. Methods From March 2017 to July 2019, 79 patients with AF who underwent surgical treatment in the Second Department of Cardiology, Puyang People’s Hospital were selected as the observation group, and 79 healthy patients during the same period were selected as the control group. Comparing the levels of serum TGF-β1, Omentin-1, Gal-3, human type Ⅰ collagen C-terminal peptide(PCIP) and type Ⅰ pro-hydroxy terminal crosslinked peptide(CITP) in patients with different degrees of atrial fibrosis, and analyzing the relationship between serum TGF-β1, Omentin-1, Gal-3 levels, AF, and the degree of atrial fibrosis and the correlation between serum TGF-β1, Omentin-1, Gal-3 levels and PCIP and CITP levels. Results The levels of serum TGF-β1 and Gal-3 in the observation group were higher than those in the control group, and the levels of Omentin-1 were lower than those in the control group(P<0.05);serum TGF-β1, Gal-3, PCIP, CITP levels in patients with high fibrosis were higher than those in patients with low fibrosis, and Omentin-1 levels were lower than those in patients with low fibrosis(P<0.05);serum TGF-β1, Gal-3, PCIP, CITP levels in AF patients were positively correlated with the degree of atrial fibrosis(P<0.05), serum TGF-β1 and Gal-3 were the risk factors for AF, Omentin-1 was the protective factor(P<0.05);Omentin-1 level was negatively correlated with the degree of atrial fibrosis(P<0.05);serum TGF-β1 and Gal-3 were positively correlated with atrial fibrosis biochemical markers PCIP and CITP(P<0.05);Omentin-1 was negatively correlated with atrial fibrosis biochemical markers PCIP and CITP(P<0.05). Conclusion The abnormal expression of serum TGF-β1, Omentin-1 and Gal-3 may be the molecular mechanism of AF induced by atrial fibrosis and atrial structural remodeling, which is related to the occurrence and maintenance of AF and can provide reference for the early diagnosis and treatment of AF.
