摘要
目的探讨侧卧位Frosch入路治疗胫骨平台后外侧骨折的具体方法及优势,为临床工作提供新的选择。方法收集2018年1月至2020年1月来自武汉市第四医院的12例胫骨平台骨折患者,术前通过影像学检查证实骨折范围累及胫骨平台后外侧。其中男8例,女4例,年龄26~59岁,平均38.6岁。均采用侧卧位Frosch入路进行骨折部位的暴露、复位及固定。术后进行规范化的功能锻炼。术后定期随访,拍摄膝关节标准的正侧位片,采用HSS膝关节评分系统评价术后膝关节功能。采用描述性统计方法,所有数据以均数±标准差(x±s)表示。结果随访时间(12±2)个月。所有患者的伤口均愈合,无感染及血管损伤等并发症。所有病例均有良好的骨折复位并最终骨愈合。HSS膝关节评分为(92±3)分。结论采用Frosch入路治疗胫骨平台后外侧骨折效果良好,可在临床上推广应用。
Objective To explore the specific methods and advantages of lateral decubitus Frosch approach in the treatment of posterolateral tibial plateau fractures,and to provide a new choice for clinical work.Methods A total of 12 patients with tibial plateau fracture from Wuhan Fourth Hospital from January 2018 to January 2020 were collected,and preoperative imaging examination confirmed that the fracture range involved the posterolateral tibial plateau.Among them,there were 8 males and 4 females,aged from 26 to 59 years,with an average age of 38.6 years.The lateral decubitus Frosch approach was used to expose,reduce and fix the fracture site.Postoperative standardized functional exercise was performed.Regular follow-up was performed after surgery.Standard anteroposterior and lateral radiographs of the knee were taken,and Hospital for Special Surgery(HSS)knee scoring system was used to evaluate postoperative knee function.The descriptive statistical method was used for data analysis.All measurement data were presented as(x±s).Results The follow-up time was(12±2)months.The wounds of all patients healed without infection and vascular injury.All patients had good fracture reduction and eventual bone union.The HSS knee score was 92±3.Conclusion Frosch approach is effective in the treatment of posterolateral tibial plateau fractures and can be widely used in clinical practice.
