Changes of accommodative function in the progression of myopia in children

摘要 目的观察眼调节功能在儿童近视进展中的变化,并探讨各参数的变化与近视度数增长之间的联系。方法前瞻性研究。选取7~12岁近视儿童患者60例(120只眼),均配戴框架眼镜矫正。分别于初次就诊时、6个月、12个月时进行散瞳验光、调节反应(BCC)、正/负相对调节(PRA/NRA)、计算性AC/A、调节灵敏度(AF)的检测。根据两次随访的近视度数(散光以等效球镜度数计算)增长的算术平均值分为增长缓慢组(<0.50 D)共24例、增长快速组(≥0.50 D)共36例。结果与增长缓慢组相比,增长快速组的BCC调节滞后值和计算性AC/A值更高,正/负相对调节(PRA/NRA)值和双眼调节灵敏度(AF)更低,其差异有统计学意义(均P<0.01),而单眼调节灵敏度(AF)的差异无统计学意义(P>0.05);BCC、PRA、AC/A与近视增长度呈负相关(P<0.01),NRA、双眼AF与近视增长度呈正相关(P<0.01),单眼AF与近视增长无相关(P>0.05)。结论学龄期近视儿童近视普遍增长较快,同时表现出调节滞后和AC/A增高,相对性调节和双眼调节灵敏度下降,且均与近视增长度具有相关性,这提示调节功能的下降是近视进展的高危因素之一。 Objective To observe the changes of eye accommodation function in the development of myopia in children,and to discuss the relationship between the changes in accommodative parameters and the increased myopic degrees.Methods This was a prospective study.A total of 60 myopic children(120 eyes)aged 7~12 years old and corrected with glasses were selected.Cycloplegic autorefraction,accommodative response(BCC),positive/negative relative accommodation(PRA/NRA),computational AC/A and accommodative facility(AF)were measured at baseline,6 months and 12 months,respectively.According to the arithmetic mean value of myopic power(astigmatism is calculated by the spherical equivalent)change during the two follow-up visits,the children were divided into two groups:the slow-growth group(<0.50 D,24 cases)and the fast-growth group(≥0.50 D,36 cases).Results Compared with the slow-growth group,the fast-growth group had higher BCC accommodation lag value and computational AC/A value,lower PRA/NRA value and binocular AF,with statistically significant differences(all P<0.01),but difference in the monocular AF was not statistically significant(P>0.05).The BCC,PRA and AC/A were negatively correlated with myopia growth(P<0.01),while NRA and binocular AF were positively correlated with myopia growth(P<0.01),and monocular AF was not correlated with myopia growth(P>0.05).Conclusions Myopia progressed rapidly among school-aged children.The accommodation lag and increase of AC/A,as well as the relative accommodation and decrease of binocular AF were all correlated with myopia progression,which suggests that the decline of accommodation function is a risk factor for myopia progression.
