摘要
目的探讨胆胃宁颗粒联合奥美拉唑肠溶片治疗湿热中阻型胃溃疡(GU)效果及对幽门螺杆菌(HP)根除率和不良反应的影响。方法选取2018年1月—2019年5月本院收治的GU患者147例,按治疗方法不同分为对照组72例和观察组75例。对照组在铋剂四联疗法后继续口服奥美拉唑,观察组在对照组基础上加用胆胃宁颗粒。比较2组治疗前后中医证候评分、HP根除率、胃黏膜形态学、氧化应激因子、炎性因子水平变化。比较2组患者不良反应发生情况与复发情况。结果观察组中医证候疗效评分显著低于对照组(P<0.05)。观察组胃镜疗效与HP根除率高于对照组(P<0.05)。与治疗前比较,2组治疗后胃黏膜形态评分和炎性因子水平显著降低,氧化应激因子水平显著升高,且观察组较对照组变化更显著,差异有统计学意义(P<0.05)。观察组不良反应发生率与复发率显著低于对照组(P<0.05)。结论胆胃宁颗粒联合奥美拉唑肠溶片对湿热中阻型GU具有较好的疗效,可有效提高HP根除率,减轻炎症反应,改善氧化应激水平,减少临床不良反应与复发。
Objective To investigate curative effect of Danweining Granules combined with Omeprazole Enteric-coated Tablets in treatment of patients with dampness-heat resistance gastric ulcer(GU)and its influence on helicobacter pylori(HP)eradication rate and adverse reactions.Methods A total of 147 patients with GU admitted during January 2018 and May 2019 were selected and divided into control group(n=72)observation group(n=75)according to therapeutic methods.Control group was treated with Omeprazole Enteric-coated Tablets orally after bismuth quadruple chemotherapy,while observation group was added with Danweining Granules on the basis of treatment for control group.In two groups,changes of Traditional Chinese Medicine(TCM)symptom scores,HP eradication rate,gastric mucosa morphology,oxidative stress factors and inflammatory factors were compared,and incidence rates of adverse reactions and recurrence conditions were recorded.Results In observation group,score of curative effect by TCM symptoms score was significantly lower(P<0.05),while gastroscopic curative effect and HP eradication rate were significantly higher than those in control group(P<0.05).After treatment,scores of gastric mucosa morphology and levels of inflammatory factors were significantly decreased,while levels of oxidative stress factors were significantly increased compared with those before treatment in two groups,and the changes in observation group were more significant than those in control group(P<0.05).Incidence rate of adverse reactions and recurrence rate in observation group were significantly lower than those in control group(P<0.05).Conclusion Danweining Granules combined with Omeprazole Enteric-coated Tablets may achieve better curative effect in treatment of patients with dampness-heat resistance GU.It may effectively improve HP eradication rate,relieve inflammatory response,improve level of oxidative stress and reduce incidence rates of adverse reactions and recurrence in clinic.
