摘要
目的探讨基于上皮-间质转化(EMT)标记的循环肿瘤细胞(CTCs)表型检测对原发性乳腺癌的预后评估意义。方法回顾性分析16例具有化疗前后两次Canpatrol CTCs分型与计数结果的乳腺癌患者的临床病理及生存资料。结果肿瘤直径越大者(P=0.021)、淋巴结有转移者(P=0.017)、TNM分期越高者(P=0.020)的化疗前CTCs数量更多;P53阳性者(P=0.003)、表皮生长因子受体(EGFR)阳性者(P=0.027)的化疗前CTCs表型以混合型为主;复发/转移者的化疗后CTCs数量较多(P<0.001),且表型以混合型为主(P=0.041);化疗后CTCs表型以混合型为主的患者总体预后较差(DFS:P=0.021,OS:P=0.061)。化疗后CTCs总数减少者的预后较好,但差异无统计学意义(P>0.05)。结论基于EMT的CTCs表型检测对原发性乳腺癌预后具有预测价值,并能为治疗方案的选择、疗效的评估提供重要信息。
Objective To investigate the implications of prognostic evaluation of circulating tumor cells(CTCs)phenotype detection based on epithelial-mesenchymal transition(EMT)markers for primary breast cancer.Methods The clinicopathological and survival data of 16 primary breast cancer patients with Canpatrol CTCs typing and counting before and after chemotherapy were retrospectively analyzed.Results Patients with larger tumor diameter(P=0.021),lymph node metastasis(P=0.017)and higher TNM stage(P=0.020)had more CTCs before chemotherapy.The prechemotherapy CTCs phenotypes of P53 positive patients(P=0.003)and epidermal growth factor receptor(EGFR)positive patients(P=0.027)were mainly mixed type.The count of CTCs after chemotherapy was higher in patients with recurrence or metastasis(P<0.001),and the phenotype was mainly mixed(P=0.041).Patients with hybrid CTCs phenotype after chemotherapy had a poor prognosis(DFS:P=0.021,OS:P=0.061).The prognosis of patients with reduced total CTCs count after chemotherapy was better,but the difference was not statistically significant(DFS:P=0.510,OS:P=0.602).Conclusion The EMT-based CTCs phenotype detection has predictive value for the prognosis of primary breast cancer,and can provide important information for the selection of treatment options and the evaluation of efficacy.
