摘要
目的:观察在常规西药干预基础上给予舒肝化浊汤治疗肝郁脾虚型非酒精性脂肪性肝炎(NASH)的临床效果。方法:将116例符合要求的患者随机分为对照组和观察组各58例。2组患者均服用多烯磷脂酰胆碱胶囊、甘草酸二铵胶囊、维生素E软胶囊治疗,观察组加予舒肝化浊汤,2组均连续治疗3个月。治疗前、治疗3个月后检测谷丙转氨酶(ALT)、谷草转氨酶(AST)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及空腹血糖(FBG)、空腹胰岛素(FINS),并计算胰岛素抵抗指数(IRI),进行腹部CT检查,评定肝郁脾虚证评分。比较2组的中医证候疗效与腹部CT疗效。结果:治疗后,观察组中医证候疗效与腹部CT疗效均优于对照组(P<0.05)。2组ALT、AST水平均较治疗前下降,观察组ALT、AST水平均低于对照组,差异均有统计学意义(P<0.01)。2组TC、TG、LDL-C水平均较治疗前下降,HDL-C水平均较治疗前升高,差异均有统计学意义(P<0.01),观察组TC、TG、LDL-C水平均低于对照组,HDL-C水平高于对照组,差异均有统计学意义(P<0.01)。2组FBG、FINS、IRI均较治疗前下降,观察组3项指标值均低于对照组,差异均有统计学意义(P<0.01)。结论:在现代医学保肝、降酶、抗氧化等综合干预基础上给予舒肝化浊汤治疗,可进一步起到护肝降酶的效果,并能调节糖、脂代谢,改善胰岛素抵抗,疗效较为确切。
Objective:To observe the clinical effect of the application of Shugan Huazhuo tang based on routine western medicine intervention for nonalcoholic steatohepatitis(NASH) of liver depression and spleen deficiency type. Methods:A total of 116 cases of patients who were in accordance with the requirements were randomly divided into the control group and the observation group,59 cases in each group. Both groups were given the oral administration of polyene phosphatidylcholine capsules,diammonium glycyrrhizinate capsules and vitamin E soft capsules;the observation group was additionally treated with Shugan Huazhuo tang. Both groups were treated for three months. Before and after three-month treatment,the alanine aminotransferase(ALT), aspartate transaminase(AST), total cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C) and fasting blood glucose(FBG), fasting insulin(FINS)were detected;the indexes of insulin resistance index(IRI) were calculated;the abdominal CT was performed;the scores of liver depression and spleen deficiency syndrome were evaluated. The curative effect of Chinese medicine syndromes and abdominal CT in the two groups was compared. Results: After treatment, the curative effect of Chinese medicine syndromes and abdominal CT in the observation group was better than that in the control group(P<0.05);ALT and AST in the two groups were decreased when compared with those before treatment, and the above two indexes in the observation group were lower than those in the control group,differences being significant(P<0.01). The levels of TC,TG and LDL-C in the two groups were decreased when compared with those before treatment, and the levels of HDL-C were increased,differences being significant(P<0.01);the levels of TC,TG and LDL-C in the observation group were lower than those in the control group, and the level of HDL-C was higher, differences being significant(P<0.01). FBG, FINS and IR in the two groups were decreased when compared with those before treatment,and the above three indexes in the observation group were all lower than those in the control group, differences being significant(P<0.01). Conclusion: Based on the comprehensive intervention of modern medicine, such as liver protection, enzyme reduction and anti-oxidation, the application of Shugan Huazhuo tang can further play the effect of liver protection and enzyme reduction,regulate the glucose and lipid metabolism,and improve IR with relatively definite curative effect.
