摘要
目的探讨醒脑开窍针刺法对脑梗死恢复期患者神经功能及脑血流动力学的影响。方法选择我院2017年6月至2019年2月收治的70例脑梗死恢复期患者,随机将其分为对照组(n=35)与针灸组(n=35)。两组患者均采用常规西药治疗,针灸组在此基础上施加醒脑开窍针刺法。比较两组的临床疗效、神经功能、日常生活能力、脑血流动力学指标及预后情况。结果针灸组的治疗总有效率为94.29%,高于对照组的77.14%(P<0.05)。治疗15、30 d时,两组患者的NIHSS评分均低于治疗前,BI评分均高于治疗前,且针灸组优于对照组(P<0.05)。治疗15、30 d时,两组患者大脑中动脉Vs、Vd、Vm均较治疗前显著升高,PI、RI均较治疗前显著降低,且针灸组优于对照组(P<0.05)。治疗60 d后,针灸组的预后良好率为65.71%,高于对照组的40.00%(P<0.05)。结论醒脑开窍针刺法治疗脑梗死恢复期患者效果显著,能显著改善患者的神经功能、日常活动能力、脑血流动力学指标及预后。
Objective To investigate the effect of Xingnao Kaiqiao acupuncture on neurological function and cerebral hemodynamics in patients with cerebral infarction in convalescent stage.Methods Seventy patients with cerebral infarction in convalescent stage treated in our hospital from June 2017 to February 2019 were randomly divided into control group(n=35)and acupuncture group(n=35).Both groups were treated with conventional western medicine,and the acupuncture group was treated with Xingnao Kaiqiao acupuncture on the basis of conventional western medicine.The clinical efficacy,neurological function,activities of daily living,cerebral hemodynamics indexes and prognosis were compared between the two groups.Results The total effective rate of treatment in the acupuncture group was 94.29%,which was higher than 77.14%in the control group(P<0.05).After 15 and 30 days of treatment,NIHSS scores of the two groups were lower than those before treatment,BI scores were higher than those before treatment,and those of the acupuncture group were better than the control group(P<0.05).After 15 and 30 days of treatment,Vs,Vd and Vm of middle cerebral artery in the two groups significantly increased,PI and RI significantly decreased,and those in the acupuncture group were better than the control group(P<0.05).After 60 d of treatment,the good prognosis rate of the acupuncture group was 65.71%,which was higher than 40.00%of the control group(P<0.05).Conclusion Xingnao Kaiqiao acupuncture in the treatment of patients with cerebral infarction in convalescent stage has significant effect,which can significantly improve the neurological function,ability of daily activities,cerebral hemodynamic indexes and prognosis of patients.
