摘要
目的:探讨不同方式联合氨基酮戊酸光动力疗法(ALA-PDT)治疗基底细胞癌(BCC)的疗效。方法:74例经病理确诊为浅表型或结节型BCC患者,分别采用手术联合ALA-PDT、高频电刀联合ALA-PDT及单纯使用ALA-PDT治疗。治疗后观察创面愈合情况和时间;1年后观察肿瘤复发、疤痕形成、外观效果等情况。结果:74例患者中,手术联合ALA-PDT治疗39例,高频电刀联合ALA-PDT治疗31例,单纯ALA-PDT治疗4例。所有患者术后创面均未发生感染,平均创面愈合时间为(12.34±1.32)d。3例单纯ALA-PDT治疗患者1年后肿瘤复发;7例出现疤痕增生,1例发生疤痕挛缩;66例(89.19%)对治疗后外观满意,8例基本满意。结论:BCC可根据皮损发生部位及面积等选择不同方式联合ALA-PDT治疗,临床安全性高且疗效较佳。
Objective:To investigate the efficacy of aminolevulinic acid-based photodynamic therapy(ALA-PDT)for basal cell carcinoma.Methods:Seventy-four patients who were pathologically diagnosed as superficial and nodular basal cell carcinoma were divided into three groups.One group was treated with surgery combined with ALA-PDT,the other group was treated with endotherm knife combined with ALA-PDT and the third group was treated with ALA-PDT alone.Following the treatments,one-year follow-up was carried out to evaluate wound healing,recurrence,scar formation and gross appearance of surgical sites.Over efficacy was assessed.Results:Among the 74 patients,39 cases were treated with surgery combined with ALA-PDT,31 cases were treated with endotherm knife combined with ALA-PDT,and 4 cases were treated with ALA-PDT alone.The average time for wound healing was(12.34±1.32)days,without wound infections observed in any patients.During the follow-up period,recurrence was found in three patients treated with the ALA-PDT group.Seven cases developed mild proliferative scar,and 1 case developed cicatricial contracture.Sixty six patients(89.19%)were satisfied with the treatment outcomes,while 8 cases were moderately satisfied.Conclusion:Basal cell carcinoma can be treated safely and effectively with a variety of methods in combination with ALA-PDT according to its location and size.
