摘要
目的:探讨三尖瓣环游离壁起源的室性期前收缩(PVCs)老年患者心电图特征。方法:选取2018年1月-2019年12月于本院行射频消融术治疗的35例三尖瓣环游离壁起源的PVCs老年患者的临床资料进行回顾性分析。根据起源点将其分为侧壁组(n=11)、前侧壁组(n=10)及后侧壁组(n=14)。比较三组术前12导联同步心电图的QRS波群特征。分析三尖瓣环游离壁起源点时钟点数与QS波或s(S)波深度、r(R)波高度的相关性。结果:35例患者均消融成功。aVF、Ⅱ、Ⅲ导联的r(R)波高度由后侧壁组、侧壁组、前侧壁组,逐渐增高,而s(S)波深度逐渐变浅(P<0.05)。aVL导联的R波高度由后侧壁组、侧壁组、前侧壁组逐渐下降(P<0.05)。前侧壁组aVR导联的QS波深度大于后侧壁组(P<0.05)。aVF、Ⅱ、Ⅲ导联的r(R)波高度与三尖瓣环游离壁起源点钟点数呈正相关,而与s(S)波深度、aVR导联的QS波深度、aVL导联的R波高度均呈负相关(P<0.05)。结论:三尖瓣环游离壁起源的PVCs老年患者经导管射频消融术治疗的成功率高,心电图特征有规律可循,掌握其体表心电图特征可为消融靶点确认提供参考。
Objective:To explore the ECG characteristics of elderly patients with ventricular premature contraction (PVCs) originated from the free wall of tricuspid annulus.Method:The clinical data of 35 elderly patients with PVCs originated from the free wall of tricuspid annulus treated radiofrequency ablation in our hospital from January 2018 to December 2019 were retrospectively analyzed.They were divided into lateral wall group (n=11),anterior lateral wall group (n=10) and posterior lateral wall group (n=14) according to their origin points.The QRS group characteristics of 12 lead synchronous ECG before operation were compared among the three groups.The correlation between the number of clock points at the origin point of tricuspid valve circumferential wall and the depth of QS wave or s (S) wave and the height of r (R) wave was analyzed.Result:Ablation was successful in all 35 patients.The height of r (R) wave in aVF,Ⅱ,Ⅲ leads by the posterior lateral wall,lateral wall,anterior lateral wall groups,gradually increased,and s (S) wave depth shallow gradually (P<0.05).The height of R wave in aVL lead decreased gradually from posterior lateral wall group to anterior lateral wall group (P<0.05).The QS wave depth in aVR lead of the anterior lateral wall group was greater than that of the posterior lateral wall group (P<0.05).The height of r (R) wave in aVF,Ⅱ,Ⅲ leads were positively correlated with the clock points of originated from the free wall of tricuspid annulus,but negatively correlated with s (S) wave depth,QS wave depth in aVR lead and R wave height in aVL lead (P<0.05).Conclusion:The success rate of catheter radiofrequency ablation in elderly patients with PVCs originated from the free wall of tricuspid annulus is high,the ECG characteristics can be followed regularly,mastering the surface ECG characteristics can provide reference for the confirmation of ablation targets.
