摘要
目的探讨膀胱肿瘤抗原(BTA)联合尿脱落细胞学检查在膀胱肿瘤诊断中的应用价值。方法收集2018年9月至2019年9月本院经膀胱镜检查或手术后病检确诊的膀胱肿瘤(37例)及非膀胱肿瘤(32例)患者的尿液标本.分别行尿脱落细胞学及BTA检测并进行相关分析。结果①尿BTA诊断前列腺肿瘤的灵敏度(81.08%)高于尿脱落细胞学(59.46%),但其特异度(62.50%)和阳性预测值(71.43%)均低于尿脱落细胞学的特异性(96.87%)和阳性预测值(95.65%),差异均有统计学意义(P<0.05)。两者联合检查的灵敏度(97.30%)高于单独尿BTA检查(81.08%)和单独尿脱落细胞学检查(59.46%),联合检查的阴性预测值(95.24%)及诊断符合率(81.16%)均高于单独检查(P<0.05);②BTA表达水平和灵敏度与肿瘤病理分级、分期无明显相关性(P>0.05)。结论BTA联合尿脱落细胞学检测可提高对膀胱肿瘤的检出率.具有较高的临床应用价值。
Objective To explore the value of bladder tumor antigen(BTA)and urinary,exfoliative cytology in the diagnosis of bladder tumors.Methods Urinary exfoliative cytology and BTA test were performed on bladder tumors(37 cases)and non-bladder tumors(32 cases)diagnosed by cystoscopy or postoperative surger in our hospital from September 2018 to September 2019.Results The sensitivity of urinary BTA group(81.08%)was higher than that of urinary exfoliative cytology group(59.46%),but the specificity(62.50%)and positive predictive value(71.43%)of urinary BTA group were lower than that of urinary exfoliative cytology group,and there were significant difference between the two groups(P<0.05).The sensitivity of the combined group(97.30%)was higher than that of urinary BTA group(81.08%)and the exfoliative cytology group(59.46%),and the negative predictive value(95.24%)and diagnostic coincidence rate(81.16%)of the combined group were higher than the other two groups.However,BTA expression level and sensitivity were not correlated with tumor pathological grade and stage(P>0.05).Conclusions The combined detection of BTA and urinan exfoliative cytology improve the detection rate of bladder tumors and have important clinical value.
