血培养标本的病原菌构成及耐药性分析

摘要 目的:分析血培养标本中的病原菌构成及耐药性。方法:收集2016年1月-2020年1月老年人群血培养标本中检测出的病原菌200例,对所有标本进行细菌构成分析及耐药性检测,分析检测结果。结果:200例病原菌中,包含金色葡萄球菌、凝固酶阴性葡萄球菌、肠球菌、链球菌、大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、鲍曼不动杆菌、其他肠杆菌,其他非发酵革兰阴性杆菌;其中大肠埃希菌数量最高。所有细菌对药物青霉素、头孢曲松、左氧氟沙星等药物均存在不同程度的耐药性。长期卧床老年人的菌株耐药性明显高于半自理老人和自理老人,差异有统计学意义(P<0.05)。结论:血培养标本中检测出的病原菌数量较多,且种类较为复杂,老年人群之间的检测数量也不相同。病原菌对药物产生的耐药性也不相同,因此临床应根据检测的病原菌构成结构,分析其耐药性,选择合适的抗菌药物。 Objective:To analyze the composition of pathogenic bacteria in blood culture specimens and the drug resistance of pathogens.Methods:From January 2016 to January 2020,200 cases of pathogenic bacteria were collected from the blood samples of the elderly,all samples were analyzed for bacterial composition and drug resistance test.Results:Of the 200 pathogens,included staphylococcus aureus,coagulase negative staphylococcus,enterococcus,streptococcus,Escherichia coli,Klebsiella pneumoniae,pseudomonas aeruginosa,Acinetobacter baumannii,other enterobacter,other non-fermentative Gram-negative bacilli.Escherichia coli has the highest number.All bacteria had varying degrees of resistance to drugs such as penicillin,ceftriaxone and levofloxacin.The strain resistance of long-term bedridden elderly was significantly higher than that of semi-self-care elderly and self-care elderly,the difference was statistically significant(P<0.05).Conclusion:The number of pathogenic bacteria that detected in blood culture specimens is large and the types are more complicated and the number is different among different elderly.The drug resistance of pathogenic bacteria to drugs is also different,so clinical should be based on the structure of the detected pathogens,analyze their drug resistance and select appropriate antimicrobial agents.
