摘要
目的:探究丙戊酸钠联合左乙拉西坦或拉莫三嗪治疗癫痫对脑电图的影响。方法:选取2014年7月至2020年3月本院收治的癫痫患者384例。按照随机数字表简单随机分为对照组、研究组1与研究组2,各128例。对照组给予丙戊酸钠治疗,研究组1给予丙戊酸钠联合左乙拉西坦治疗,研究组2给予丙戊酸钠联合拉莫三嗪治疗,观察三组患者治疗后的发作控制率、脑电图疗效、认知功能的差异。结果:发作控制方面,治疗后研究组1、2癫痫发作控制率明显高于对照组,组间和时点的差异均有统计学意义(P<0.05)。脑电图疗效方面研究组1与研究组2总有效率显著高于对照组(P<0.05)。治疗前,三组P300潜伏期、MoCA评分比较差异无统计学意义(P>0.05);治疗后研究组1、研究组2 P300潜伏期、MoCA评分水平均高于对照组,差异有统计学意义(P<0.05)。结论:丙戊酸钠联合左乙拉西坦或拉莫三嗪治疗癫痫均具有较好疗效,能有效控制癫痫发作,减少放电、改善脑电图背景,提高认知功能。临床应根据患者具体情况合理选择联合用药。
Objective:To investigate the effects of sodium valproate combined with levetiracetam or Lamotrigine in the treatment of epilepsy on eeg.Methods:A total of 384 epileptic patients admitted to our hospital from July 2014 to March 2020 were selected.According to the random number table,the patients were randomly divided into control group,study group 1 and study Group 2,each of 128 cases.The control group was treated with sodium valproate,the study group 1 was treated with sodium valproate combined with levetiracetam,and the study group 2 was treated with sodium valproate combined with lamotrigine.The differences of attack control rate,eeg efficacy and cognitive function among the three groups were observed.Results:In terms of seizure control,its rates of study group 1 and 2 after treatment were significantly higher than that of control group,and the differences between groups and time points were statistically significant(P<0.05).The total effective rate of study group 1 and study group 2 was significantly higher than that of control group(P<0.05).Before treatment,there were no statistically significant differences in P300 incubation period and MoCA scores among the three groups(P>0.05);after treatment,the P300 incubation period and MoCA scores of study group 1 and study group 2 were higher than those of control group,and the differences were statistically significant(P<0.05).Conclusion:Valproate combined with levetiracetam or lamotrigine in the treatment of epilepsy has a good effect in effectively controlling seizures,reducing electrical discharge,improving eeg background,improving cognitive function.The combination drug should be selected reasonably according to the specific situation of the patients.
