摘要
目的研究四维超声联合二维超声检查对孕中期疑似心脏畸形胎儿的诊断价值,为临床诊断提供依据。方法选择2017年1月-2019年10月在聊城市东昌府区妇幼保健院接受产前检查的2250例孕妇作为本次的研究对象。按照随机数字表法,将研究对象平均分为研究组与对照组,各1125例。对照组产妇的检查选择美国GE公司生产的彩色超声检查仪器,凸阵式二维超声探头频率为3.5 MHz进行。研究组产妇的检查在对照组基础上选择RAB2-5四维超声探头,频率为2.5~7.0 MHz。以出生后的复查结果为参照,比较两组的诊断准确率、胎儿的心脏畸形筛查的准确性、特异度及灵敏度。结果研究组的诊断准确率(93.75%)明显高于对照组的诊断准确率(63.33%),差异有统计学意义(χ^(2)=20.7539,P=0.0012)。研究组诊断特异度与灵敏度(93.31%、95.13%)均明显高于对照组诊断特异度与灵敏度(88.54%、82.16%),差异有统计学意义(P<0.05)。结论四维超声联合二维超声检查在胎儿心脏畸形辅助诊断中应用价值较高,能够对胎儿心脏结构异常细节进行清晰显示。
Objective To research the value of four-dimensional ultrasonography combined with two-dimensional ultrasonography in diagnosis of fetuses suspected of cardiac abnormalities during the second trimester of pregnancy,provide a basis for clinical diagnosis.Methods From January 2017 to October 2019,2250 pregnant women receiving prenatal examination were selected and divided into study group and control group according to random number table,1125 pregnant women in each group.The pregnant women in control group underwent two-dimensional ultrasonography(3.5 MHz),and the pregnant women in study group underwent four-dimensional ultrasonography combined with two-dimensional ultrasonography(2.5-7.0 MHz).Taking the reexamination results after birth as reference data,the accurate rates of diagnosis,accuracies,specificities,and sensitivities of screening of fetal cardiac abnormalities in the two groups were compared.Results The accurate rate of diagnosis in study group was 93.75%,which was statistically significantly higher than that in control group(63.33%)(χ^(2)=20.7539,P=0.0012).The specificity and sensitivity of screening in study group were 93.31%and 95.13%,respectively,which were statistically significantly higher than those in control group(88.54%,82.16%)(P<0.05).Conclusion The application value of four-dimensional ultrasonography combined with two-dimensional ultrasonography in auxiliary diagnosis of fetal cardiac abnormalities is high,the method can clearly display details of fetal cardiac structural abnormalities.
