摘要
目的研究双球囊导管促宫颈成熟在延期妊娠引产中的应用效果及对妊娠结局的影响。方法前瞻性选择2016年1月至2019年12月安徽医科大学附属宿州医院产科收治的80例延期妊娠产妇作为研究对象,采用随机数字表法将其分为2组:对照组40例采用传统的缩宫素引产,研究组40例采用双球囊导管引产。比较2组宫颈成熟度、引产效果、分娩方式、分娩结局、产后出血、新生儿窒息等情况。结果研究组产妇临产前Bishop评分高于对照组,临产时间短于对照组,研究组1 min、5 min新生儿Apgar评分均高于对照组,差异有统计学意义(P<0.05)。研究组阴道分娩率为85.00%,明显高于对照组的62.50%,差异有统计学意义(P<0.05)。研究组第一产程、总产程短于对照组,差异有统计学意义(P<0.05)。2组新生儿出生时体重、产妇并发症和新生儿并发症发生率差异均无统计学意义(P>0.05)。2组第二产程、第三产程和总产程差异均无统计学意义(P>0.05)。结论双球囊导管促宫颈成熟操作简单,引产效果好,可提高自然分娩率,缩短临产时间和产程,且不会增加并发症,可推广应用。
Objective To study the effect of double balloon catheter for cervical ripening in the induction of delayed pregnancy and its effect on pregnancy outcome.Methods Eighty cases of postpartum pregnant women in Department of Obstetrics from Suzhou Hospital Affiliated to Anhui Medical University January 2016 to December 2019 were selected as the research object.They were divided into two groups using the random method of the digital table.A double balloon catheter was used to induce labor,and the cervical maturity,labor induction effect,delivery method,labor outcome,postpartum hemorrhage,and neonatal asphyxia were compared between the two groups.Results The maternal pre-natal Bishop score of the study group was higher than that of the control group,and the time of labor was shorter than that of the control group.The Apgar scores of newborns in the study group at 1 min and 5 min were higher than that of the control group,and the difference was statistically significant(P<0.05).The vaginal delivery rate in the study group was 85.00%,which was significantly higher than 62.50%in the control group,and the difference was statistically significant(P<0.05).The first stage and total stage of labor in the study group were shorter than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in birth weight,maternal complications and incidence of neonatal complications between the two groups(P>0.05).There was no significant difference in the second stage,third stage and total stage of labor between the two groups(P>0.05).Conclusion The double balloon catheter can promote cervical ripening,which is easy to operate and has a good induction effect.It can increase the rate of natural delivery,shorten the time and duration of labor,and will not increase complications.It can be popularized and applied.
