摘要 目的:对比体外冲击波碎石术(ESWL)和输尿管镜钬激光碎石术(URL)急诊治疗输尿管中下段结石的临床疗效。方法:2018年6月-2019年12月收治输尿管中下段结石患者110例,按不同治疗方式分为两组。所有患者均在急诊初诊12 h内接受急诊ESWL和急诊URL;对照组采取ESWL治疗;观察组采取URL治疗。比较两组临床疗效。结果:观察组平均治疗时间、术后疼痛缓解率、碎石成功率及2周结石排净率均优于对照组,差异有统计学意义(P<0.05)。结论:急诊ESWL和急诊URL均是治疗输尿管中下段结石的安全有效方法,但急诊URL疗效优于急诊ESWL,尤其是直径>10 mm的结石。 Objective:To compare the clinical efficacy of extracorporeal shock wave lithotripsy(ESWL)and ureteroscopic holmium laser lithotripsy(URL)in the emergency treatment of middle and lower ureteral calculi.Methods:From June 2018 to December 2019,110 cases of patients with middle and lower ureteral calculi were enrolled,they were divided into two groups according to different treatment methods.The emergency treatment of ESWL and URL were received by all patients within 12 h of first aid.The control group was treated with ESWL,while the observation group was treated with URL.The therapeutic effects between the two groups was compared.Results:The average treatment time,postoperative pain relief rate,lithotripsy success rate and 2 weeks of stone removal rate in the observation group were better than the control group,the differences were statistically significant(P<0.05).Conclusion:ESWL and URL are safe and effective methods in the emergency treatment of middle and lower ureteral calculi,but URL is better than that of ESWL,especially for those with diameter of more than 10 mm.
