摘要
目的:研究银杏达莫注射液联合高压氧对重型颅脑损伤术后患者认知功能和血清NSE、NGB表达的影响。方法:按照随机数字表法将2018年6月至2019年10月期间我院收治的128例重型颅脑损伤患者分为对照组和研究组(n=64),所有患者采用改良去大骨瓣减压术治疗后,对照组患者行高压氧(舱内压力设置为0.2 Mpa)治疗,研究组患者在高压氧治疗基础上增加银杏达莫注射液15 ml静脉滴注治疗,2次/d,持续治疗3个月。于治疗3个月后评估两组患者临床疗效;于治疗前及治疗3个月后使用格拉斯评分(Glasgow coma scale,GCS)量表、简易智力状态检查量表(Mini-mental State Examination,MMSE)、蒙特利尔认知评估量表(Montreal Cognitive Assessment,MOCA)评估患者认知功能;治疗前及治疗3个月后采用酶联免疫吸附法测定血清神经元特异性烯醇化酶(Neuron-specific enolase,NSE)、人脑红蛋白(Neuroglobin,NGB)含量,采用后免疫比浊法测量患者血清中丙二醛(Malondialdehyde,MDA)、超氧化物歧化酶(Superoxide dismutase,SOD)含量。结果:与治疗前相比两组患者治疗后GCS评分、MMSE评分、MoCA评分均显著升高,其中研究组更为显著(P<0.05);与治疗前相比两组患者治疗后NSE、NGB含量、MDA含量均显著降低,且研究组更为显著(P<0.05);与治疗前相比两组患者SOD含量均显著升高,且研究组更为显著(P<0.05)。结论:银杏达莫注射液联合高压氧可显著降低重型颅脑损伤术后患者的氧化应激水平,缓解神经组织损伤,促进认知功能的恢复。
Objective:To study the effect of ginkgo damo injection combined with hyperbaric oxygen on cognitive function and serum NSE and NGB expression in patients with severe craniocerebral injury after surgery.Methods:According to the random number table method,128 patients with severe craniocerebral injury in our hospital from June 2018 to October 2019 were divided into the control group and the study group(n=64).All patients were treated with modified decompressive craniectomy.The control group was treated with hyperbaric oxygen(cabin pressure was set at 0.2 MPa),and the study group was treated with ginkgo leaf extract and Damo injection 15 times on the basis of hyperbaric oxygen The treatment lasted for 3 months.The clinical efficacy of the two groups was evaluated after 3 months of treatment;Glasgow Coma Scale(GCS),Mini Mental State Examination(MMSE)and Montreal Cognitive Assessment(MCE)were used before and after 3 months of treatment The levels of serum neuron specific enolase(NSE)and Neuroglobin(NGB)were measured by enzyme-linked immunosorbent assay before and 3 months after treatment,and malondialdehyde(MDA)and superoxide dismutase(superoxide dismutase)were measured by post immunoturbidimetry SOD content.Results:Compared with before treatment,GCS score,MMSE score and MOCA score of the two groups were significantly increased after treatment,especially in the study group(P<0.05).Compared with before treatment,the levels of NSE,Ngb and MDA in the two groups were significantly decreased after treatment,especially in the study group(P<0.05).Compared with before treatment,the content of SOD in the two groups were significantly increased,and the study group was more significant(P<0.05).Conclusion:Ginkgo Damo injection combined with hyperbaric oxygen can significantly reduce the level of oxidative stress in patients with severe craniocerebral injury,relieve nerve tissue damage,and promote the recovery of cognitive function.
